AFFIDAVIT

I, PO2 KEN OSEO HONRUBIA, of Filipino, of legal age with address at Blk 7 Lot
12 Rosa Verde Villas, San Rafael, Rodriguez, Rizal, after having duly sworn to in
accordance with law, do hereby depose and state:

1. That a Memorandum of Explanation dated October 17, 2017 was issued

directing the me to explain in writing his failure to appear in the scheduled
court hearing before the Municipal Trial Court on case Nos. 1705-130, 1705-131,
and 1705-132;

2. That on the scheduled time and date, I experienced severe itching on different
parts of my body due to chronic inflammatory disease called Psoriasis, a
condition of the skin that gets better and worse spontaneously.

3. After few hours of putting an ointment and orals intake medicine, the itching
finally subsided. Being already late on the said court hearing (arraignment), I
went directly my duty at COMPAC 3, LUVERS Barangay San Isidro,
Rodriguez, Rizal because I was informed that PO1 Florante Bogbog already
attended the said court Municipal Trial Court Hearing.

4. That this affidavit is being executed to attest to the truth based on the foregoing
facts and not intended to delay any court proceedings.
