AFFIDAVIT

I, LOUIE L. SANTOS, Filipino, of legal age, single, and presently residing at No. 51 E. Rimando
Street, Ortiz, Naguilian, La Union, after having been sworn in accordance with law do hereby depose
and say:

1. I am an associate attorney at Reynaldo Cortes Law Office with office address at Rm. 307 Jose
Miguel Building, No. 1 Labsan Street, Baguio City;

2. That in the morning of June 7, 2017, I together with Ms. Sharron Manuel, the secretary of our law
office, served a copy of the Notice of Dishonor and Demand Letter for Payment to the Spouses Jose
Celestino and Nedda Villacin at their residence at No. 21 Libra Street, Bel-Air Village, Barangay Bel-Air,
Makati City (A photocopy of the Notice of Dishonor and Demand Letter for Payment is attached
hereto as Annex “A”);

3. That at the aforementioned address, I was able to speak with Ms. Susan Delos Santos, a person of
sufficient age and discretion and a person living with the spouses at their residence, and told her that
I am delivering a Notice of Dishonor and Demand letter for Payment to the spouses;

4. That the copy of said Notice of Dishonor and Demand Letter for Payment was refused to be
received by Susan Delos Santos who told me that the spouses were not at home and that they left
early that morning, and that she was under instruction from the spouses that she should refuse to
accept letters from Atty. Reynaldo A. Cortes or Therese Jison;

5. That because of the refusal, I left the copy of the Notice of Dishonor and Demand Letter for
Payment in their mail box and told Ms. Susan Delos Santos that I left the copy in there;

6. That we were accompanied by members of the Tanod of Barangay Bel-Air and a representative of
Bel-Air Security Office of Bel-Air Village Association, Inc. in serving the said Notice of Dishonor and
Demand Letter for Payment. Barangay Bel-Air and the Security Office of Bel-Air Village Association,
Inc. then issued us copies of their Excerpt Reports pertaining to the service of the Notice of Dishonor
and Demand Letter for Payment to the spouses Jose Celestino and Nedda Villacin. (Photocopies of the
Excerpt Reports of Barangay Bel-Air and the Security Office of Bel-Air Village Association are attached
hereto as Annexes “B” and “C”, respectively);

IN WITNESS WHEREOF, I have hereunto affixed my signature this _____ day of June 2017 at
