The following are the requirements to be submitted by the interested applicants:

Mandatory Requirements:
a. Letter of intent which shall indicate the following information:
1. Statement of purpose / expression of interest
2. Subject group he/she intends to teach
3. Preferred school(s), if any
b. CSC Form 212. Revised 2005 (Personal Data Sheet) in two (2) copies with latest 2x2
ID pictures
c. Certified photocopy of certificates of relevant specialized trainings, if any
d. Certified copy of Voter’s ID and/or any proof of residency
e. National Bureau of Investigation (NBI) Clearance
f. Omnibus certification of authenticity and veracity of all documents

Additional Requirements:
a. Certified photocopy of Diploma on Bachelor’s Degree
b. Certified photocopy of Transcript of Records with at least 15 units of specialization in
relevant strand/specialized subject
c. Certified photocopy of Professional Regulation Commission (PRC) professional ID
card/certificate of registration/license
d. Certified photocopy of ratings obtained in the Licensure Examination for Teachers
(LET) / Professional Board Examination for Teachers (PBET)
e. Applicants for Part-Time Positions must submit a written approval from his/her head
of unit if he/she is currently employed by the national government or the local
government unit

The applicant shall submit to the head of elementary or secondary school where a teacher
shortage or vacancy (regular and/or natural) exists, a written application, with the Applicant
Number indicated, supported by the following documents:
a. CSC Form 212. Revised 2005 (Personal Data Sheet) in two (2) copies with latest 2x2
ID pictures
b. Certified photocopy of PRC professional identification card or a PRC certification
showing the teacher’s name, LET rating, and other information recorded in the PRC
Office
c. Certified photocopy of ratings obtained in the Licensure Examination for Teachers
(LET) / Professional Board Examination for Teachers (PBET)
d. Certified photocopy of Transcript of Records/CAV
e. Copies of service records, performance rating, and a school clearance for those with
teaching experience; if unavailable, the applicant must submit a justification citing
the reason/s for unavailability
f. Certified photocopy of certificates of relevant specialized trainings, if any
g. Certified copy of Voter’s ID and/or any proof of residency
h. National Bureau of Investigation (NBI) Clearance
i. Omnibus certification of authenticity and veracity of all documents
