Healthy People 2020 Goal

Healthy People 2020 states, “Improve access to health care services”

(www.healthypeople.gov).

Short Term Goal

To develop a council consisting of nurses and doctors who are willing to give their

times and ideas towards the process of establishing an Urgent care center/ER.

Long Term Goal

To establish an Fully equipped Urgent Care/ ER.

Outcome Criteria

 With in three months, we will have a public meet in Poland to dicusses

funding and planning for the new urgent care/ER.

 By 6 months, the funding will be collect and the blueprint for the facility will

be started. Construction site and date will be choose.

 After eighteen months, the Urgent Care/ER will be established and all Nurse/

Doctors/Aids will be hired. And the opening date will be established.

 By twenty-four months, specific training for the Nurses/ Doctors/ AIDS of

Poland will be completed. All resources are available to the staff members to

utilize equipment safely and efficiently. The official date will be open to the

public.

Nursing Diagnosis

Risk for traumatic Injuries to result in death for the people of Poland Ohio due to the

lack of Urgent Care/Er within their city limits.

As Evidence, By

 The people of Poland are lacking a close triage center.

 The city of Poland must travel to the Boardman or Austintown Emergency

department for emergency medical treatment.

Interventions

 The community will come together to raise awareness about how quickly it

takes for a death to occur from a traumatic injury.

 We will start a petition for the new implementation for the new urgent

care/ER.

 We will talk to people who are in the high-risk category to have certain

medical complication such as: Diabetics, Heart problems, Bleeding disorders.

 We will contact the state health department to establish an appropriate

funding for the urgent care/ER.

Rationales

 Often people aren’t sure what’s wrong and wait to long before getting help.

This is to raise awareness to get help quickly.

 Many people die or are disabled from a heart attack, cardiac arrest or a

stroke because they do not get lifesaving treatment in time. Every second

wasted is heart or brain tissue lost, increasing the risk for death or disability.

 Per the Ohio State Health Department their institute is the proper route to

access for appropriate funding community facilities. (Funding Opportunities,

June 13, 2017).

Evaluation

Three-Month Evaluation

By three months we had a meeting the funding for the urgent care/ ER

Six-Month Evaluation

By six months the council was developed, a list of equipment needed to provide

services was established and documented, and affiliation with hospitals was

established and documented.

The closest hospital with emergency services, St. Elizabeth’s downtown

Eighteen-Month Evaluation

By eighteen months, all staff was hired and all the equipment needed was purchased

for the emergency service. The current residence of Poland ohio were all mailed

brochures and papers informing them of the new facilities address, phone number

and services that will be available.

Twenty-four Month Evaluation

By twenty-four months, training courses were completely for all paramedics. The

Poland Urgent Care/ ER was finally open and 750 residence from various

communities and 500 of their own residence.
