Dear Mr. Black:

Pursuant to the Ohio Open Records Act (Ohio Rev. Code Ann. Sec. 149.43 to 149.44), I am
writing to request information related to Assistant Chief Dave Bailey's departure from the police
department.

+ Please provide all documents dating back to March 5, 2018 that detail the reason Bailey left the
department.

+ Please provide the settlement offer first given to Bailey and the final settlement agreement.

+ Please provide Bailey's personnel file.

I plan to use this information for a news story, and therefore, ask for your prompt response to this
request and immediate access to this information. Because this information will be used to
inform the public, I ask that you waive any copying fees. If there will be a charge and that charge
exceeds $25, I ask that you disclose the sum of the copying before producing the documents or
other items.

If any part of my request is denied, please notify me in writing, citing the specific policies and/or
statutes that allow you to deny my request. Where exemptions to the act are discretionary, I ask
that you make the records available even if they might qualify for withholding under the law.

Also, if a particular part of the record I am requesting becomes available before other parts of it,
please fulfill my request in part and continue to collect the remainder.

While I would prefer this information in electronic form, I will accept the record in the format
that is most readily available and delivered to me via the fastest method available. That includes
coming to a city office to inspect the records, since some of them may be on paper.
