Introduction
While Operating Company’s wells are a significant asset they are also a significant potential
liability.
To ensure that Operating Company’s wells’ risks are as low as reasonably practicable all company
wells’ designs, their drilling plans and operations are examined by a competent independent well
engineer referred to as a Well Examiner. While this may be a regulatory requirement in some
countries Operating Company have adopted the practice to ensure that all well drilling and
completion operations
Although Operating Company ensures it employs competent drilling engineers and supervisors, it is
the Project Drilling Manager who ensures this initially as he is accountable to Operating Company
for the well designs, plans and operations. However, the potential impact of well or drilling failure
is so significant Operating Company apply two further mitigations to identify and correct any
omissions or errors;
• Operating Company’s Drilling Technical Authority (who is independent of both the project
team and asset) reviews all well designs and plans and receives all Daily Drilling Reports
(DDRs). The close proximity provides early response and reaction.
• Furthermore Operating Company engages a competent industry authority to examine its
well designs, plans and operations. As this external verification is independent of the
project, asset and Operating Company’s well engineering and operations function it
reassures both company and industry regulators about the reliability of Operating
Company’s wells and drilling operations.
The Well Examiner applies a standard methodology sometimes referred to as a “Written Scheme of
Verification” and qualifies the design or plans. NB: As operator Operating Company retains liability
for the well, its design and plans, consequently the examination does not certify the well, its
designs or plan.
At the instruction of Operating Company Ltd Well Examination may also be implemented in the
Company’s well operations conducted in locations other than the UKCS.
Regulation 18 of the UK’s Design and Construction Regulations requires well operators to have a
written scheme of examination (referred to as the "Arrangements for Examination" in the
regulations), to ensure that wells are “designed, commissioned, constructed, equipped, operated,
maintained, suspended and abandoned so that as far as is reasonably practical there can be no
unplanned escape of fluids from the well and that the risk to the health and safety of persons from
it, anything in it, or in the strata to which it is, are as low as reasonably practicable”.
It is a requirement of the DCR regulations that well examination shall be undertaken by an
Independent and Competent person who is sufficiently knowledgeable and separate from the
immediate well operators line management.
The Operating Company scheme of Well Examination employs risk based principles allowing the
independent Well Examiner to use his/her skills, experience and knowledge to vary the degree of
examination required during well design, well construction and well operations to ensure that the
appropriate measures have been employed to reduce risks to as low as reasonably practicable
(ALARP) levels.
Reasonably practicable is a narrower term than "physically possible" and implies a computation
must be made in which the quantum of risk is placed on one scale and the sacrifice whether in
money, time or trouble involved in the measures necessary to avert the risk is placed in the
other. If it can be shown that there is a gross disproportion between them and that the risk is
insignificant in relation to the sacrifice, then Operating Company Energy as the duty holder
discharges the burden of proving that compliance was not reasonably practicable.

In addition to fulfilling Operating Company Energy requirements to comply with the DCR
regulations the Scheme offers the following additional benefits:
• It provides data for peer reviews and audit processes;
• It can be used by Joint Operating Partners to review and monitor the performance of
Operating Company Energy;
• It provides quality assurance of well related regulatory submissions; and
• It allows technical review by input from an experienced well engineer.
The Arrangements for Examination shall be subject to periodic review and revised as appropriate.

of Well Examination is available for the induction new staff by following the attached link.
All activities highlighted in bold using the word “shall“ are considered to be mandatory.

2.0 Well examination process


2.1. General
The objective of the well examination scheme is to provide the necessary assurance to Operating
Company Ltd that all wells for which it is the nominated well operator are adequately designed,
constructed, maintained and operated throughout their lifecycle.
The scheme shall clearly demonstrate that
• the pressure boundary or envelope of a well is monitored and managed throughout its life
cycle; and
• any pressure containment equipment that forms part of the well is suitable for its purpose.
Details of the examination scheme and sufficient well examination records shall be kept to form an
auditable trail. These shall record
• what aspects have been examined;
• when the examination was performed;
• how the findings and recommendations were communicated to the Well Engineering
Teams; and
• what changes were implemented as a result.
Where a well is associated with a platform there may be an overlap between the well examination
scheme and the verification scheme of an installation. The nature of this relationship is depicted in
Appendix 1 Figure 1.
Roles and responsibilities of the main personnel involved in well examination and the verification
scheme of an installation are detailed on the RACI chart depicted in Appendix 1 Figure 2 and are
outlined as follows:
Well Examiner
• runs the well examination scheme;
• performs the actual examination work;
• when requested by Operating Company Ltd participates in HAZID and HAZOP activities
associated with well programming activities;
• compiles End of Operation's Reports (see Appendix 2 for examples of Well Examination Sheets
produced) and maintains the scheme’s records;
• delivers reports to the QHSE Manager
• is an independent and competent person;
• has a minimum experience level of 10 years well engineering and/or operational well design
and construction experience;
• has provided documented evidence of competency;
• is supplied by an independent external contractor, and reports to the QHSE Manager thus
providing independence from the Operations Teams (see Appendix 1 Figure 1); and
• is not involved in the well operation being examined in any other capacity.

The well examiner is responsible for


• the continuing examination of the well pressure containing envelope of both subsea and
platform wells in accordance with the Operating Company Ltd Scheme of Well
Examination. This will include all associated materials and equipment contained within
and upon the well.
• the regular review of well activities to ensure that wells are designed, constructed,
equipped, maintained and operated with their safe operating envelope.
• The regular review of any well related dispensations relating to well integrity permitting
the continued operation of both platform and subsea wells.
Note: For subsea wells this includes well equipment upstream of and including the
production wing valve including all wellhead, tree valve and SCSSVs but does not
include subsea flowlines.
Examination activities will include:
• Review of wellhead, tree valve and SCSSV integrity test records to check that function and
pressure tests are being performed to the appropriate Operating Company Energy policies
and to the appropriate performance standards.
• Ensure any follow up actions arising from failure to meet the performance standards are
properly closed out.
• Check that regular ROV inspections are being performed as specified by the appropriate
Operating Company Energy policies and to the appropriate performance standards.
• Review of well related reports.
Offshore Supervisor
• is responsible for ensuring that the well operation is performed as specified in the programme;
and
• ensures that adequate data is included in the Daily Operations Report to allow completion of
the Construction Examination Sheet.

Drilling Engineer
• provides the data (design documentation, Daily Drilling Reports etc) to the Well Examiner to
enable the examination of the design and construction phases to be carried out;
• provides responses to any queries raised and to any formal Comment Sheets issued by Well
Examiner; and
• informs the Well Examiner of any changes to the programme, and/or dispensations from policy
that might be issued.

Manager Well Operations


For all well activities shall:-
• provide the data (design documentation, Daily Operations Reports etc) to the Well Examiner to
enable the examination of the design and construction phases to be carried out;
• inform the Well Examiner of any changes to the programme, and/or dispensations from policy
that might be issued.
• brief the Offshore Supervisor about the well examination scheme;
• provide the Offshore Supervisor with a Construction Summary Sheet prior to the work so that
he/she knows what information must be included in the Daily Operations Report;
• be responsible for ensuring timely responses to any Comment Sheets issued by the Well
Examiner;
• be responsible for the day to day management of the Well Examiner;
• ensure that all well examination documentation is correctly forwarded to the appropriate
personnel as defined in Appendix 1 Figures 2, 3 and 4; and
• ensure that all well examination documentation is correctly filed in accordance with DCR
Regulation 18 (4).
• maintain a listing of UKCS wells for which Operating Company Ltd Limited is accountable and is
to be regarded as persuasive in other regimes which have no specific regulatory requirements
for well consent/verficataion. This listing shall be constantly updated to reflect changes to well
status or additions and deletions to the listing arising from new well suspension, old well
abandonments, acquisitions, trades and relinquishments;
It should be noted that liabilities for abandoned and suspended wells remain the responsibility of
the licence holder.
QHSE Manager
• provides a reporting point for the Well Examiner which is independent of the operations line;
• resolves any disagreements between the Well Examiner and the Well Operations Team by
involving the Managing Director to mediate any final decision.;
• is responsible for ensuring that End of Operations reports are filed in a clearly identified and
secure location; and
• ensures the HSE is informed of the location at which the well examination records are kept.
All Personnel involved in the Scheme
Periodic Well Examination review meetings shall be held as required to allow all those involved in
the scheme (as depicted in Figures 1-6 Appendix 1) to propose, discuss, and implement any
changes deemed necessary.

The Well Examiner shall examine the well design to ensure that it complies with Operating
Company Ltd policies and procedures and that it enables well integrity to be maintained during the
construction and operation phases of the well. In performing this examination the Well Examiner
shall require access to various documents such as: -
• Well proposal or basis of design;
• Rig Move Procedures;
• Installation Operations Safety Case(s) (and Combined Operations Safety Case(s) - if
applicable);
• Shipping Survey (if applicable);
• Casing Design Document;
• Details of offset data used;
• HSE Notification;
• Material changes to HSE Notification;
• Site Survey and Shallow Hazard Assessment;
• Well Programme and Amendments if applicable;
• Minutes of any risk assessment, HAZID, or pre-operational meetings;
• Expected well operating parameters;
• DTI Application to Drill (PON 4/WONS submission);
• DTI Application for workover/intervention (PON8/WONS submission);
• Dispensations from policy;
• Handover certificates; and
• DTI Application to abandon/suspend (PON5/WONS submission).
A Well Design Examination Sheet shall be completed to provide an auditable record of the
examination.
If the Well Examiner has any queries or concerns that are not answered in a timely manner to
his/her satisfaction by verbal or e-mail communication then a formal Comment Sheet (see example
shown in Figure 14 Appendix 2) shall be issued to the Manager Well Operations whose Team is
responsible for the design.
The Manager Well Operations issued with the formal Comment Sheet shall respond to the
concerns raised by completing the relevant section of the form and return it to the Well Examiner
within an appropriate timeframe.
The Well Examiner shall maintain a register of all e-mail correspondence exchanged and any
formal Comment Sheets issued and closed out.
In cases of dispute where the comments from the Well Examiner are not accepted and there is a
‘failure to agree’, the Well Examiner shall raise the matter with the QHSE Manager who will
involve the Managing Director to mediate any final decision.
Copies of any amendments to the work programme shall be sent to the Well Examiner for review
and details entered on the Design Examination Sheet. The Design Examination Sheet will
consequently remain open until the well operation is completed.

After examining the well design the Well Examiner shall produce a Construction Examination
Sheet, which details, in the form of a checklist, the safety critical operations, which will be
performed during the well construction process.
As the well progresses the Well Examiner shall complete the checklist to confirm that the well has
been constructed according to the design, or, if not, what action has been taken to maintain well
integrity.
During the Construction Phase the Well Examiner shall require access to certain documents, such
as: -
• Activity Reports (e.g. Daily Drilling Reports, Casing and Cementing Reports, Perforating
reports, BOP test reports etc.);
• Copies of completed Handover Certificates;
• Final Well Schematic;
• Final Wellhead Schematic, if applicable;
• Seabed Clearance Certificate;
• OIR9B Reports - if applicable (well integrity related only); and
As with the Design Phase, the Well Examiner will engage in verbal or e-mail communication and
can issue formal Comment Sheets if there are any comments or queries during the Construction
Phase.

2.4. Workover phase (REF. PROCESS FLOW CHART IN


APPENDIX 1 FIGURE 4 & 5)
The examination of workovers shall be performed using a similar process as already described for
drilling operations. However, routine operations from a fixed platform, which require the removal
of Xmas tree swab cap, (e.g. PLT or similar survey) do not need to be formally examined for each
entry. Where such operations are carried out under standard procedures, the Well Examiner shall
examine these procedures on a one-off basis.
Operating Company Energy shall supply hard copies of such standard procedures to the Well
Examiner for examination. Once examined the Well Examiner will confirm in writing that they have
been examined and they should be entered onto the list of documents in Appendix 3.
Well operations from a fixed platform, which need neither DTI consent nor HSE notification and do
not require removal of pressure containing well components, do not require examination.

2.5. Operational phase (REF. PROCESS FLOW CHART IN


APPENDIX 1 FIGURE 6)
The DCR requires a well examination scheme to be in place throughout the entire life cycle of each
well, including the operational (i.e. production/injection) phase. Examination of such wells shall
entail an initial examination of the well's operating and maintenance procedures, and regular
monitoring of data provided in operational reports (e.g. annulus pressures, DHSV and tree valve
testing records, maintenance records etc.).

2.6. Suspension phase (SEE FLOW CHART OF PROCESS IN


APPENDIX 1 FIGURE 6)
Should it be necessary to temporarily suspend an operational well, then the suspension process
shall be examined in a similar manner to that used for a workover operation as described above.
Should a new well be suspended this shall be examined as part of the drilling examination.
Where wells are suspended for extended periods of time then it shall be necessary to examine
how these wells are maintained and monitored.

2.7. Abandonment phase


When it becomes necessary to permanently abandon a well, the abandonment process shall
require examination. As with suspension, if this is an operational well, then the abandonment
shall be examined in a similar manner to that used for a workover operation as described above.
Should a decision be made to permanently abandon a new well, the abandonment shall be
examined as part of the drilling examination.
Once a well is permanently abandoned it is no longer necessary to maintain or monitor the well
and it is no longer necessary for the well to be covered by the Well Examination Scheme.
It should be noted that liabilities for abandoned and suspended wells remain the responsibility of
the licence holder.

2.8. End of well examination report (SEE EXAMPLES IN


APPENDIX 2)
After the well operation is finished the Well Examiner shall produce an End of Well Operation
Examination Report, which shall be the formal Operating Company Ltd auditable record to
demonstrate examination of the design and construction of the well.
This report shall comprise, as a minimum:-
• copies of the completed Design and Construction Summary Sheets;
• supporting documentation;
• closed out Comment Sheets;
• copies of any dispensations from policy issued; and
• a register of all e-mail correspondence exchanged.
The Well Examiner shall send the end of operation report to the QHSE Manager for sign off and
formal filing thereby completing the Well Examination process for the well operation in question.
Operating Company Ltd shall retain the completed End of Operation Well Examination Report
(including all examination and comments/query sheets and other supporting correspondence) in
compliance with DCR Regulation 18 (4).

2.9 Bi annual integrity report (SEE EXAMPLE IN FIGURE 13


APPENDIX 2)
Every two years (bi-annually) the Well Examiner shall produce a well integrity review sheet for
each well for which Operating Company Ltd is the nominated well operator. This shall be copied
to the well owner and any issues associated with continued well integrity clearly identified. If a well
integrity issue is identified the well owner shall respond to the well examiner with details of the
remedial action planned and the proposed timetable for implementation.

2.10 Scheme review


The agreed processes for Well Examination shall be subject to periodic review as required.
The purpose of the review shall be to
• confirm the proper working of the examination scheme;
• identify areas where it could be improved;
• review comments raised during well examinations to identify any trends in the type of
deficiencies found, so that any remedial action can be put in place; and
• review planned and actual changes to the various policies, procedures , codes and
standards as listed in Appendix 3 to determine whether any changes are required.
Records of meetings held to review the scheme shall be written up by the Well Examiner and
retained on file by Operating Company Ltd. These records shall document any agreed actions, the
parties responsible for completing the action and subsequent close out.

3.0 Operating company responsibilities under the scheme


Operating Company shall provide the Well Examiner with an up to date listing of the various
policies, procedures, codes, and standards that it has formally approved to control well and well
related activities. This listing defined in Appendix 3 shall form the document set that will be used
as a basis for the well examination. Operating Company Ltd shall provide the Well Examiner with
copies or access to copies of Operating Company Ltd policies and procedures.
Operating Company Ltd shall ensure that the Well Examiner is updated when there are changes
made to this listing or any of the documents listed.
These policies, procedures, codes, and standards include (but are not limited too):
- Well Construction Policy & Procedures;
- Casing Design Policy & Procedures;
- Well Barrier Policy & Procedures;
- Well Control Policy and Procedures;
- Well Operation and Maintenance Procedures;
- Well Integrity Policy and Standards; and
- API, NACE, UKOOA, IP, and other Industry Codes of Practice and
Standards.
The various policies, procedures, codes, and standards that Operating Company Ltd has formally
approved shall be applied to all stages of well design, construction, operation, and maintenance.
If, at any time, this does not occur then a properly authorised dispensation shall be issued and
copied to the Well Examiner.
During the planning phase of the well, Operating Company Ltd shall ensure all reasonable steps to
obtain predictions of the subsurface environment, which can be expected in the well from offset
well data. Where such information is limited Operating Company Ltd shall identify potential worst
case conditions, and shall identify all potential hazards and circumstances likely to lead to unsafe
well conditions. This shall include not only formations which may pose a hazard directly but also
those which may affect the ability to control a hazardous situation e.g. loss zones, shallow gas,
H2S, stuck pipe, over pressured zones, plastic formations etc. For such circumstances mitigating
actions shall be put into place for each hazard identified.
During well design it shall be the responsibility of the person(s) planning the well to ensure that
selection of materials shall be made on the basis of their fitness for use in the downhole
environment and loading conditions likely to be experienced during drilling, completion, workover,
suspension and abandonment activities.
Only Contractors with an acceptable capability and competency with installations and equipment
meeting the appropriate safety standards shall carry out construction of wells. Such work shall
be carried out under the supervision of a Operating Company Ltd wellsite representative and all
stages of construction and commissioning shall be fully reported to the Operating Company Ltd
Manager Well Operations and monitored/reviewed against the previously agreed well plan.
Information returned from the wellsite during construction of the well shall be used to assess
actual conditions against those parameters used in the design and deviations notified to the Well
Examiner. In order to facilitate well examination the Offshore Supervisor shall ensure that all the
necessary data on the items critical to well integrity, which are listed in the Construction Sheet, are
included in the Daily Operations Reports.
Any problems, programme changes or differences between planned and actual operations, which
may impact on well integrity, shall be highlighted to the Well Examiner by the Manager Well
Operations either via the Examination Construction Sheet or by agreed channels of communication.

If the well plan has to be amended for any reason it shall be done using Operating Company Ltd's
approved management of change procedures.
All equipment used in Operating Company Ltd wells during construction, intervention and
operational activities shall be fit for its intended purpose and shall comply with Operating
Company Ltd approved specifications listed in Appendix 3 and meet appropriate industry
standards. Prior to commencement of any well operation Operating Company Ltd shall have
confirmed with the Installation Owner that safety critical items of equipment available on the
installation (e.g. blow-out preventers, choke manifolds, degassing equipment, wireline lubricators)
are suitable for the pressures and flow rates expected during the well operation.
Wells shall be operated in accordance with the appropriate Operating Company Ltd documented
procedures. All controls necessary for the safe operation of a well shall be in place and fully
functioning or the necessary dispensations in place. Tree valves and sub-surface safety valves
shall be cycled and tested at a frequency established from risk and reliability data in line with
Operating Company Ltd approved procedures to ensure that they meet the desired performance
standard.
Well design shall always take into consideration the need for future suspension or abandonment.
The design shall be such that suspension or abandonment can be completed in a safe manner in
accordance Operating Company Ltd approved policies and that there shall be no unplanned
escapes of fluids thereafter.
