Age Division Rules and Registration Requirements

1. November 6, at 12:00a.m.of the current year, shall be the determining date of the
age of the player and Age Division in which the player in eligible to participate.
2. Eligibility to participate is based first on the age determining date. The grade of
the athlete is not a consideration if the athlete meets the age determining date.
3. Teams must submit roster with all required information before beginning
tournament play complete with parent signature, if the player is under eighteen.
4. Teams electing to protest eligibility of a player or players on a team must do so
prior to the start of a game.
5. Protest will not be accepted once a game begins.
6. If a player is found to be ineligible that player will be disqualified from further
participation in the tournament. The player’s team may continue in tournament
play without the player.
7. Registration deadline is October 31, 2010.
8. Registration fee is $30 per team. Mail Registration, Roster and Entry Fee (Check
or Money Order Payable to Tyler Street UMC)
9. Age Divisions are as follows:
Children: 8 –12 years
Teens: 13-17 years
 Rookies: 18-40 years
Veterans: 41+

Medical Waiver and Release of Liability (This form must be signed by the parent/
guardian of each player before player is eligible to participate in tournament )
I, the above signed, hereby authorize any first aid, medication, medical treatment or
surgery deemed necessary in case of an emergency for the above player during Three on
Three Basketball Tournament at TSUMC tournament play. I, the above signed, in
consideration of the players participation in Three on Three Basketball Tournament at
TSUMC tournament, intending to be legally bound, do hereby ourselves, executors, and
administrators waive, release, and forever discharge any and all rights and claims for
damages, including any claims for loss, damages or injury to our persons or property
arising out of the above player’s performance or failure of performance from the Three
on Three Basketball Tournament at TSUMC, their agents, representatives, successors
and assigns.

I certify that the information within is correct to the best of my knowledge. I
understand that should a protest arise concerning the eligibility of any players
participating on my team, that it will be necessary that proper documentation (e.g.
Birth Certificate) be made available verifying the player’s eligibility in the age
group in which that player is participating. It is understood that should one of my
players be found ineligible, that the player will not be able to continue participating
in the tournament.
