Effect of Collaborative Care System (CCS) on Blood Glucose Levels
in Type 2 Diabetes Mellitus Outpatient
Base on Kuman, N. & Rahajeng, B. (2016) states that type 2 diabetes
mellitus is a metabolic disorder disease in which there is damage to the pancreas
organ precisely on beta cells that function to produce insulin, usually the disease
is characterized by the state of hyperglycemia resulting from the least secretion of
insulin by beta cells. In the world have done various actions to overcome health
problems, especially in type 2 diabetes mellitus but the action has not been able to
solve the health problems. One of the actions is to apply a collaborative care
system where the implementation of this collaborative care system in patients
with type 2 diabetes mellitus desese will be able to manage well. Collaborative
care system is a system of health care interventions in providing primary care to
patients using case management by incorporating three health problems in patients
with type 2 diabetes mellitus including medical, pharmaceutical and nursing care.
In this journal conducted research on collaborative care system where the purpose
of this study to determine the effect of collaborative care system on blood glucose
levels of patients with type 2 diabetes mellitus.

This study used quasi-experimental design method, is there was a group of


intervention is group of patients with type 2 diabetes mellitus given collaborative
care system at Fakfak Health Center and control group is group of patients with
type 2 diabetes mellitus who were not given collaborative care system at Balai
Pengobatan Misi, data were obtained by measuring blood glucose levels in
patients with type 2 diabetes mellitus obtained a large sample of 66 people with
type 2 diabetes mellitus desease. The samples were then divided into two groups,
namely the intervention group of 35 patients and the control group of 31 patients.
The data obtained were then analyzed using paired sample t test statistic
conducted in the intervention group and in the control group was tested by
Wilcoxon whereas differences in blood glucose levels between controls group and
interventions goup were compared with the Mann-Whitney test.

From the results of the study, in the intervention group of collaborative


care system for 1 month showed that there was a decrease of blood glucose level
when the mean before collaborative care system was 276,11 mg/dl and after
collaborative care system was 222,43 mg/dl. While in the control group there was
an elevated blood glucose level whereas before collaborative care system was
217,32 mg/dl and after CCS was 266,45 mg/dl. Statistical tests showed a
significant difference (p <0.05) between blood glucose levels in the intervention
group and control group before and after collaborative care system. Concluded
that from this study that the collaborative care system affect the decrease in blood
glucose levels in patients with type 2 diabetes mellitus.
