6-501.11 - PHYSICAL FACILITIES - GOOD REPAIR
Observation: Tile floor in walk-in cooler has deep crevices between tiles where grout is not sufficient. Significant
water and food debris between tiles causing bad odor throughout cooler.
Corrective Action(s): Maintain the physical facilities so they are in good repair at all times. Correct By: 02-

6-501.19 - CLOSING TOILET ROOM DOORS
Observation: (CORRECTED DURING INSPECTION): Toilet room doors are not kept closed.
Corrective Action(s): Keep toilet room doors closed except during cleaning and maintenance operations. Action
Taken: Discussed with person in charge, do not prop open bathroom doors.
