FRESHMAN ADMISSION AND ENROLLMENT PROCEDURE


ACADEMIC YEAR 2018-2019

Step 1. SUBMISSION OF ALL REQUIRED CREDENTIALS FOR ADMISSION

Submit all the REQUIRED CREDENTIALS FOR ADMISSION to the Admission Officer, Office of Admission Services
(OAS/OUR), Ground Floor, W-107 West Wing, Sta. Mesa, Manila, on the date of your scheduled processing of
credentials, interview and enrollment as reflected in your SAR Form 1.

 Only applicants with complete credentials required for admission will be accommodated and shall go through
the admission process.
 Freshman applicants are advised to come for interview in appropriate attire and proper grooming (with collar
and sleeves; no rubber slippers, ‘tokong’/shorts; and no earrings for male).

 Admission of qualified applicants is on first come, first served basis. Non-appearance on your scheduled date
of interview would mean automatic forfeiture of your schedule. However, late applicants may be re-scheduled
or put on the waiting list subject to the availability of slots.

 Non-SHS applicants whose English final grade is below 80%, proceed to the Department of English, College of
Arts and Letters, 2nd Floor North Wing Main Building, A. Mabini Campus, Sta. Mesa, Manila to be scheduled for
the English Placement Test (EPT). A fee of P150.00 shall be paid to the PUP Cashier.

Step 2. COLLEGE INTERVIEW AND REGISTRATION

Proceed to the College offering the program of your first choice for interview by the Dean or Chairperson. In case you
are not qualified or there is no more available slot in your first choice, you can go to the college of your second or third
choice until you get the approval for acceptance in your desired program.

Admission in PUP is on a FIRST-COME-FIRST-SERVED basis subject to the availability of slots.

In addition to the general criteria set by the University, admission to the program depends on the satisfaction of the
specific criteria set by the Colleges for AY 2018-2019 and in passing the department interview.

Step 3. MEDICAL EXAMINATION

Proceed to the PUP Medical Clinic, Ground Floor, East Wing Main Building, A. Mabini Campus, Sta. Mesa, Manila, and
present your Student Admission Record Form 1 (SAR F1) to secure a medical clearance. Your chest X-Ray film, not older
than six (6) months is required.

Step 4. ISSUANCE OF REGISTRATION CERTIFICATE

Submit your accomplished SAR Form 1 to the PUP Admission Services, Rm. 107 Ground Floor, West Wing Main Building,
A. Mabini Campus, Sta. Mesa, Manila to get the printed copy of your Registration Certificate, which will be announced
during the enrollment period. Proceed to your assigned Admission Staff base on the first letter of your surname.

NOTE: Present your Registration Certificate to your teachers during the first day of classes.

Step 5. ISSUANCE OF STUDENT ID

Proceed to the Office of the Student Services at Rm 208, Charlie Del Rosario Hall, A. Mabini Campus, Sta. Mesa, Manila
for the processing of your Student ID application after the issuance of your Registration Certificate. Schedule will be
announced by the Office of the Student Services. Always visit the PUP Website and PUP official social media channels.

First Semester Classes will start on June 18, 2018.

FRESHMAN ADMISSION AND ENROLLMENT PROCEDURE

WHAT ARE THE ADMISSION REQUIREMENTS TO BE SUBMITTED?

A. Required Credentials:
The applicant must submit an original copy and photocopies of the following documents to the Office of Admission Services:

1. For Senior High School Graduates


1.1. Original Senior High School Card (F138) or certified true copy with school dry seal

1.2. Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation

1.3. Senior High School Diploma

1.4. Original PSA Birth Certificate

1.5. Certification of Good Moral issued by the HS/SHS Principal/or Guidance Counselor with school dry seal

1.6. School ID (for currently enrolled students) or any valid ID with picture (for previous SHS graduates)

1.7. Chest X-Ray result with Film

1.8. Medical Clearance issued by the PUP Medical Clinic

1.9. Two (2) pcs. 2 in. x 2 in. colored picture with white background with applicant’s name on the photo

1.10. Original Copy of Grade 10 High School Card or Certified True Copy with school dry seal

2. For Non-Senior High School Graduates


In addition to the above admission requirements:
2.1. Certification from high school/senior high school registrar with school dry seal and noted by the Principal that no
copy of F137A has been sent to other college or university

2.2. Notarized certification of non-enrollment for high school/senior high school graduates who have not enrolled in
any technical/diploma/degree program immediately after graduation with "waiver" that if there is concealment of
previous enrollment, PUP enrollment becomes null and void

2.3. For PEPT qualifier, a duly authenticated or verified copy of the Certificate of Rating and a Certification issued by the
Department of Education (CMO No.15, s. 2003)

2.4. Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC –BAL for
secondary level

B. Other Requirements:
1. One (1) piece long, brown envelope

2. For non-SHS applicants whose English final grade is below 80%, proceed to the Department of English, College of Arts and
Letters, 2nd Floor North Wing Main Building, A. Mabini Campus, Sta. Mesa, Manila to be scheduled for the English
Placement Test (EPT). A fee of P150.00 shall be paid to the PUP Cashier.

3. For entrance scholars, Entrance Scholarship Committee Route and Approval Sheet duly signed by the members of the
concerned committee and one (1) photocopy of the credentials submitted for scholarship grant.
