ADMITTING DIAGNOSIS
Bilateral hallux valgus.

DIAGNOSTIC DATA ON ADMISSION:


WBC 6.5, RBC 4.53, hemoglobin 14.1, hematocrit 41, platlets 26, 100.
Urinalysis, clear yellow with specific gravity 1.025, pH 5, glucose negative,
ketones negative, bilirubin negative. Preoperative chest film normal.

HOSPITAL COURSE
Patient responded to anesthesia uneventfully. Postoperatively patient was
hemodynamically stable, afebrile. Physical therapy was instituted during
hospital stay. Therapist taught the patient to use crutches. Patient did well
postoperatively and with the physical therapy.

DISCHARGE DIAGNOSIS
Bilateral hallux valgus, corrected.

DISCHARGE INSTRUCTIONS
He was discharged on post-operative day 5 in improved condition to be seen
in the office for dressing check in 1 week. He was giving Tylenol No. 3 p.r.n.
for pain. Diet regular. Activities: Crutches, no weight bearing, keep lower
limbs iced and elevated as much as possible. Patient voiced understanding of
the plan as described above. Both he and his parents agreed to the plan and
scheduled follow-up.
