ACCREDITATION OF DENTAL TRADERS/EXHIBITORS

Objectives:

To promote harmonious relationship between the Exhibitors and the Philippine Dental
Association

TERMS AND CONDITIONS

1. Application for accreditation is available at PDA. The application for accreditation is a

binding contract between the exhibitors and PDA. The organization has the right to accept
or reject the application Accreditation for period of one (1) year from June 1 of the current
year to May 31 of the following year.
2. Requirements:
a. License to Operate (LTO) as Medical Devices Importer
b. BIR Registration Forms (0605)
c. Certificate of Registration , form 2303
d. Mayors Permit
e. Municipal License
f. SEC Registration for Corporation/DTI Registration for Single Proprietorship
g. Product exclusivity/distributorship from the principal supplier.
h. BFAD certificate of approval of each product that they are distributing.
3. Certificate of Accreditation will be released upon approval.
4. Only accredited exhibitors/traders are allowed to participate in all PDA, Area Convention,
Regional Conferences, Chapter Inductions, and Seminars.
5. Accredited traders will get a percent (5%) discounts on a one time full payment of the
total cost during the booth selection of every PDA Annual Convention.
6. Any problems to be encountered between Dentists and accredited traders will be handled
by the PDA through Complaints and Grievance Committee.
7. Only accredited traders are allowed to participate/sponsor the chapter activities like
induction and seminars.
8. Termination and Suspension
a. any violations of the terms and conditions
b. In the event of bouncing checks during the payment of reserved booth.
c. No subleasing policy.
d. Selling of dental materials to non-dentists
e. Participation of any accredited traders in conventions/seminar of which coincide with
the PDA activities.
9. All the rights are reserved by the PDA
