APPEAL

Comes now, ANICETA S. QUIROS, the undersigned appellant, pro se, and unto this Honorable
PNP RETIREMENT AND BENEFITS ADMINISTRATION SERVICE (PRBS), most respectfully state, that:

MATERIAL FACTS

1. The late P/Sgt CARLITO M. QUIROS who died on January 17, 1991 at AFP Medical Center in V.
Luna Road, Quezon City, was in the active service when he met his untimely death, and served
with untarnished record in the Integrated National Police (INP) service from 16 April 1970 to 17
Jan 1991 and for reference the copy of DPRM issued Service Record is attached herewith;

2. PSgt QUIROS was survived by his wife, herein appellant ANICETA S. QUIROS (hereinafter Mrs.
Quiros), and children namely: Aniceto, Carmela, Chona, Carlito Jr. and Isaac, all surnamed
Quiroz, who were still minors at the time;

3. With the supervening of RA 6975, where PSgt Quiros was absorbed to PNP, and being a
uniformed personnel, his heirs are entitled to the death or permanent incapacity of said
personnel, as provided for under Section 76, of the said Act, and/or other existing laws.

4. Mrs. Quiros started gathering all the documents pertinent to the death claims and funeral
benefits, at the time, an application for claim was filed to the HEADQUARTERS PHILIPPINE
CONSTABULALRY/INTEGRATED NATIONAL POLICE BENEFIT CLAIMS ASSISTANCE CENTER in Camp
Crame, hence, by virtue of the submitted copy of affidavit of adjudication, a recommendation,
dated 13 February 1991, for approval of benefit claims to the heirs of the late PSgt Quiros, and
designation of his heirs as legal beneficiaries, dated 16 December 1992;

5. While it was unclear, however, to Mrs. Quiros whether she filed an application form for survivor
benefits and pension claims since, she was certain that she have filed an application for burial
assistance and reimbursement of the funeral expenses she incurred for the interment of PSgt
Quiros;

6. On 5 December 1997, she declared In her affidavit that her government issued Identification
Documents, as herein attached which forms part hereof;
7. In April 2018, was her last update of her application for survivorship and pension claim due her
late husband; at the PNP- PRBS she was directed to furnish appeal accompanied with the
documents for such delayed filing for claims;

Hence, the instant appeal.

ISSUE

The sole issue to resolve is WHETHER OR NOT the heir of the late PSGT CARLITO M.
QUIROZ INP is barred to receive the survivorship benefits and pension claim.
