AFFIDAVIT OF LOSS

I, ____________, , Filipino, single, and a resident of, ____________,

after being duly sworn according to law, hereby depose and state that:
1. I am a holder of the SCHOOL ID issued by the ____________;
2. Sometime May 6, 2018, I misplaced my school ID while inside the
campus;
3. Despite diligent effort exerted, I could no longer locate, find, or recover
the said school ID, and to the best of my knowledge, the same is truly
lost;
4. The same school ID has not been confiscated by any officer of the law or
issuing office or agency due to violation of any law, statute, order, rule
or regulation;
5. In the event that the lost school ID should hereafter be found or
located, I undertake to forward or surrender the same to the
appropriate office for proper disposal;
6. I am executing this Affidavit to attest to the truth of the foregoing
statements and to request for the issuance of a new school ID in lieu of
the one which was lost.
