TO WHOM IT MAY CONCERN:

I authorize my daughter, SUZELLE Q. QUINICIO to act on my behalf in relation to the transaction

of the PSA birth certificate of my daughter, Desa Rhaine Q. Quinicio. I also authorize her to make any
necessary decisions as for the release of the PSA birth certificate.
