A. Present Health History:

A case of 45 years old, female patient was admitted due to two month PTA onset of “on and off”
chest pain: mild to moderate in intensity localized on the mild-sternal ova associated the with episodes

B. Past Health Status

General Health:
Patient was admitted for the first time in the month of September, 19 th day in year 2010. She
may sometimes feel irritable due to the chest pain that she’s experiencing.

The patient loses her teeth at the upper front area and has no dentures.

Childhood Illness:
The patient has experienced fever, cough, common colds and chicken pox in her childhood.

The patient doesn’t remember about her immunizations.

Major Illnesses/Hospitalizations:
This would be the first major hospitalization of the patient.
