INSTRUCTIONS
To be eligible, you must be a registered undergraduate. For more information on double majors and eligibility criteria, please go to
http://web.mit.edu/doublemajor. This form cannot be submitted electronically, but you may type your information into the form and are
encouraged to do so.
1. The CoC requires signatures from your current advisor and either an administrator or a faculty advisor from your intended second
major. Departmental signature requirements vary; confirm requirements with both your primary and second major.*
2. Make THREE copies of the signed application: one for you and one for each department.
3. Send the original to: Committee on Curricula, 5-115. (See deadline under Eligibility Requirements, below.)

ELIGIBILITY REQUIREMENTS TO APPLY FOR A DOUBLE MAJOR


ƒ You must be a registered undergraduate who has completed at least three terms at MIT, including at least one term in a department with a declared major. If
you are a transfer student, you must complete at least two terms of work at MIT, at least one of which must be in a department with a declared major.
ƒ You must submit your application on or before the Add Date of your penultimate term.
ƒ You must have a cumulative GPA of 4.0 or higher. Although not encouraged, students with a lower GPA may apply. The CoC will consider these applications
on a case-by-case basis at its discretion; additional documentation may be required. Write to curricula@mit.edu for more information.

Note: The Committee will consider late applications at its discretion only under extenuating circumstances.
