APPLICATION FOR BIR ACCREDITATION AS - TAX AGENT / TAX PRACTITIONER

1. Accomplish BIR Form 1916 (Attach – “A”).

2. Certified True Copy (CTC) – Certificate of Registration with BOA.
Note: Registration with BOA must be done before BIR accreditation.
3. CTC – PRC ID (Secure certified true copy from PRC).
4. Certificate of Membership in Good Standing with PICPA Chapter (Secure from PICPA).
5. Certificate of Good Moral Character issued by two (2) disinterested persons, who are either
member of the BAR or CPA in good standing (Attach – “B”).
6. A written undertaking under oath to preserve working papers within the period prescribed
under Section 235 of NIRC. (Attach – “C”).
