1. What do you consider to be the outstanding accomplishments of this child during


the past three or four years? Why did you select these as the most important?

Rylee has been a successful student attending a middle college program where she not
only completes her high school level courses but college level courses as well. She also
works part time at an after school program, a job that she secured all on her own, and
works occasionally on the weekends for a family friend’s business. Even with these
responsibilities, she still finds time to be social and have fun. I selected these as the most
important accomplishments because she is already taking steps to make her future bright,
while still maintaining a good balance in life and doing things that make her happy.

2. In what areas has this child shown the most development and growth during the
past three to four years?

The areas that Rylee has grown and developed the most in are her maturity and
responsibility. She has developed into a young adult who makes decisions, thinks about
her actions, and cares for others.

3. What do you consider to be his/her outstanding personality traits?

Rylee is charismatic and fun to be around. She is the type of person who brings a room to
life because she is very sociable and bubbly. She is also a caring person who puts her
friends and family before herself.

4. What are the best five adjectives that describe this young man/woman?

Beautiful, playful, intelligent, mature, independent

5. Are there any unusual or personal circumstances that have affected this child’s
educational experience?

Two and a half years ago when Rylee was 15, our father was diagnosed with Stage 4
cancer. He quickly declined and passed away four and a half months later. This was very
devastating for our entire family, as we are all very close with each other and with our
parents. Rylee had to process and deal with an immense amount of grief while carrying
on with her schooling. She had to adjust to a new normal and take on additional
responsibilities within our family as she navigated through the loss of a parent at such a
young age.
