An Unusual Presentation of a Vitreous Hemorrhage


A 38-year-old woman with poorly controlled diabetes
presented with gradually decreasing vision to hand motion in
the right eye. Slit lamp dilated exam revealed a crescentic
boat-shaped hemorrhage in Berger’s or Erggelet’s space (Fig
1). This retrolental vitreous hemorrhage was characterized by
an acute bleed topped by a layer of dehemoglobinized blood.
Moderate diffuse vitreous hemorrhage prevented elucidation
of fine retinal details on ophthalmoscopy, allowing only the
observation of panretinal photocoagulation scars; no large
retinal tears were evident. Dilated fundus examination of the
left eye revealed neovascularization of the optic disc with
clinically significant macular edema and neovascularization of
vessels along the inferior arcade. The ophthalmologic findings
were consistent with vitreous hemorrhage as a complication of
proliferative diabetic retinopathy.
