Administrator Verma:
Its with a great sense of consternation that Lam writing you today regarding an ongoing situation
in my district. 1am refering to Wentworth Rehabilitation and Health Care Center which CMS
thas determined warrants addtional inspections and oversight, Despite this need for additional
‘oversight and as reported in the Chicago Tribune', te issues at Wentworth do not seem to have
abated.
‘These issues appear to meet the textbook definition of fraud and substandard are, which harm
‘ott individual patients and the health care system, at large. Therefore, I am calling on CMS to
‘immediately terminate funding for this facility due to its reckless and eallous disregard forthe
Fnealth, welfare, and lives ofits residents
tis my belief that we are here today because of CMS" decision to delay enforcement of protections
for Medicare and Medicaid beneficiaries in nursing fucilities. As the August 24, 2017 Office of
Inspector General report showed, CMS “has inadequate procedures to ensure that incidents of
potential abuse or neglect at skilled nursing facilities are identified and reported in accordance with
applicable requirements.”
Accordingly, request that you provide information for the following question
1 When did CMS first lear of issues at Wentworth?
2. What corrective measures did CMS mandate from Wentworth? When were they
‘completed and how was tis verified”

4, What are the criteria used for CMS" rating system?
5. What penalties, ifany, ae issued for noncompliance with CMS mandates and misleading

‘Thank you for your attention to this matter. I look forward to your reply.
