Analysis of Week 1 and 2 Progress Notes
1. This patient has progressed to performing slide board transfers from bed to wheelchair
with minimum assistance instead of moderate assistance. This patient has also
successfully practiced stand-pivot transfers in therapy with moderate assistance from two
people. Prior to the week one progress note, the patient could stand with assistance from
two people for no more than 30 seconds. Now the patient can stand between 30 and 60
seconds. The patient has also improved his ability to self-correct his alignment and
positioning in standing and sitting.
2. This patient successfully completed his goal of safely performing functional transfers
with maximum assistance and 50% verbal cues. The patient has also been compliant with
his in-bed exercise program.

The patient continues to struggle with the following goals:


a. Long Term: Patient will safely perform bed mobility tasks with modified
independence and occasional verbal cues for proper sequencing and for proper
positioning in order to get in/out of bed and participate in edge of bed activities.
b. Long Term: Patient will safely perform functional transfers with supervised assist and
occasional verbal cues for proper sequencing, for initiation/termination of tasks and
for push up from arms of chair with ability to right self to achieve/maintain balance in
order to return to prior level of functional abilities.
c. Short Term: Patient will stand for longer than one minute using parallel bars with
Max A with proper foot and hand placement.
d. Long Term: Patient will safely ambulate on level surfaces 100 feet using parallel bars
with CGA with functional loading response and with proper heel strike 95% of the
time to return to prior living and supervision levels.
3. The patient shows good progress towards his goal of performing bed mobility tasks with
modifies independence. He will need to continue to increase his core strength and upper
body strength to accomplish this goal.
In order to perform functional transfers with supervised assist, stand for longer than one
minute, and ambulate 100 feet, the patient will need to continue to strengthen his lower
extremities. He is currently unable to take any steps once standing because of knee pain
and because of his difficulty pushing his knees back and hips forward while standing.
The patient will need to overcome his knee pain and strengthen his legs enough to lock
his knees while standing and maintain an upright posture.
