I’m Ms. Klarisse D. Madrid, wrote this letter of appeal in behalf of my leave effective this
June01, 2018 until September 2018. I’m going to attend my review in Manila and prepare myself until
my board exams in September. Before I submit my leave letter to your office, I ask first a permission to
my subordinates; all medtech on duty regarding my plan for review. I am very happy because they
always give their support.

Furthermore, if I passed my board exam on September, may I have a permission to continue my

career as a RMT in your laboratory? I promise that I do my best to prove that I am deserving of having
that position if that will happen.
