TRACHEOSTOMY CONVERSATION

One day in nursing academy, 2B.1 class is having medical surgical nursing
lecture. Today they will learn about Tracheostomy. In here Andri as the lecturer
will give explanation about tracheostomy.

Andri : Good morning everybody....

Collegers : Morning, Sir...

Andri : Ok, everyone today we will study about tracheostomy. Have all of you
heard about tracheosromy?

April : Hmm, I think tracheostomy is a operation that is done in trachea.

Andri : Is there any opinion?

Deni : Tracheostomy is a tool that put on trachea

Afrida : How is the tool put on?

Andri : Ok, we will discuss it. Tracheostomy is a surgical procedure to create


an opening through the neck into the trachea (windpipe).

Afif : How about the procedures ?


Dwi : The trachea will be put on a tube

Andri : A tube is usually placed through this opening to provide an airway and
to remove secretions from the lungs

Dede : what is the name of that tube ?

Andri : It’s called tracheostomy tube or a trach tube

Deni : What kind of patient that put on tracheostomy tube?

Andri : Patient with A large object blocking the airway

Deni : According to the book that I read, tracheostomy will be put on to the
patient with an inherited abnormality of the larynx or trachea

Andri : Is there any opinion?

Afrida : Beside that tracheostomy will be put on to the patient has a cancer of
the neck, which can affect breathing by pressing on the airway

Dede : Does the tracheostomy has risk?

Andri : Yes,the risks are bleeding, infection, Nerve injury, including paralysis.

April : Only that sir?

Dwi : According to the internet, the risks are to damage to the thyroid gland,
erosion of the trachea (rare), Puncture of the lung and lung collapse.

Andri : Exactly right .

Afif : Who is put on the tracheostomy? doctor or nurse ?

Andri : Doctor will put on the tracheostomy and nurse will take care it.

Afif : Oh..yeach sir...I want ask... How the patient will communicate to
others?
Andri : Most patients need 1 to 3 days to adapt to breathing through a
tracheostomy tube. And it may be impossible for the patient to talk or
make sounds. After training and practice, most patients can learn to talk
with a tracheostomy tube. Patients or family members learn how to take
care of the tracheostomy during the hospital stay.

Ayu : Sir... How long the treatment of tracheostomy and what equipment is
needed ?

Andri : Usually... it’s long enough time because we have to care wtih be
carefully in the treatment and tools need to be prepared to perform a
tracheostomy is :

Suction catheter, Sterile gloves, Appropriate catheter sizes, sterile and


clean and disinfected strap, sterile gauze, swab, hydrogen
peroxid,normal saline, tracheal cannul, brush, sterile bowl, protective
coat, bib tracheostomy, eye protection scissors, pedestal and towels..

Afif : Oh...yeach.. sir.. How many types of tracheostomy?

Andri : Ok... I have explained the question of you and now... I wish you were
explaining of Afif question.?

Dede : Hmm... Sir.. I want to try to answer of question it.. Ok... There are three
types of traceostomy, it’s there :

1. Surgical trakeostomy
This type can be temporary and permanent and is done in the
operating room. Incision is made between the second and third
tracheal rings 4-5 cm long.
2. Percutaneous Tracheostomy
This type is only temporary and is done in the emergency
department. Cincing be making a hole between the trachea and two
or two and three. Because the hole is made smaller, then the wound
healing will be faster and leaves no scar. In addition, the incidence of
the onset of infection is also much smaller.
3. Mini tracheostomy
Incision in the middle of the cricothyroid membrane and mini
tracheostomy was inserted using wire and dilator.
