AFFIDAVIT OF SERVICE

I, ANGEL GABRIEL DE JESUS III, as Liaison Officer, of APSSaT Law Office with office
address at, CU01A Alicante Tower, Marquinton Condominiums, Sumulong Hi-way cor. Toyota
Avenue, Barangay Sto. Niño, Marikina City after having been sworn in accordance with law,
depose and say:

1. That on 6 February 2016, around 3:00 in the afternoon. I was given a task to deliver the
Notice to Vacate to Spouses Ivy abon and Bernabe Abon Jr. at the house and lot owned
by Firebird Home Development Corporation at Hilltown Residences, Lot 7, Block 4,
Champaca Avenue, Barangay Fortune, Marikina City. I was accompanied by Mr. Kyrie
Cayanga, a liaision officer of Firebird Home Development Corporation;

2. We were faced with a person who claims of be the mother of Ms Ivy Abon. She refused
to give her name and to receive the said Notice to Vacate;

3. The following day, 7 February 2017, 2:40 in the afternoon. I and Mr. Kyrie Cayanga
again tried to serve the same Notice to Vacate to Spouses Abon. However, the same
person who claimed to be the mother of Ms Ivy Abon, shouted expletive at us and told
us to never come back. Immediately, we went to the Barangay Hall to report the refusal
to receive the said Notice to Vacate Attached herein as Annex “A” is a copy of the
Blotter Report;

4. That on the same date, 7 February 2017, I deposited the same Notice to Vacate through
Registered Mail to Spouses Ivy abon and Bernabe Abon Jr. at Hilltown Residences, Lot
7, Block 4, Champaca Avenue, Barangay Fortune, Marikina City particularly by
depositing to Marikina Post Office, as evidenced by Registry Receipt No. RD 716 815 237
ZZ and indicated after the name, and with instruction to the postmaster to return the
mail to the sender after ten (10) days if undelivered. Attached herein as Annex “B” is
the Registry Receipt.

5. In addition, on 7 February 2017, I served copies of the same Notice to Vacate through
Registered Mail to Spouses Ivy Abon and Bernabe Abon Jr. at Hilltown Residences, Lot
7, Block 4, Champaca Avenue, Barangay Fortune, Marikina City particularly by
depositing to LBC Marikina Plaza, as evidenced by Official Receipt MRK
0110000038760. Attached herein as Annex “C” is the Official Receipt.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 10th day of February
