JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, EUGENIO M. LANORIAS, 86 years old, Filipino, widower, and a resident

of Brgy. Ipil II, Palompon, Leyte; and TEODORO V. AREVALO, 77 years old, Filipino,
and resident of Brgy. Ipil III, Palompon, Leyte, after having been duly sworn to in
accordance with law, do hereby depose and jointly says, That:

We personally know VERGILIO COBACHA MERCADO who was born on May

31, 1957 at Palompon, Leyte, to parents Sps. Alfredo Naguit Mercado and Estrella
Aragon Cobacha. We know this fact because we were close family friends and
neighbors;

The birth of VERGILIO COBACHA MERCADO was duly registered at the

National Statistics Office (NSO), but because the said child was often sick and as has
been his parents’ beliefs, they decided to call him “ALFREDO COBACHA MERCADO,
JR. ” - the name that he has been commonly known in his neighborhood and in the
community, and as stated in most of his official records, including but not limited to,
his Social Security System (SSS) Records, Senior Citizen ID Card, Voter’s ID,
Scholastics Records, Marriage Contract, Employment Records, Pag-IBIG Records, and
the Certificate of Birth of his child, Justine Kirsty L. Mercado;

The names VERGILIO COBACHA MERCADO as found in his Certificate of Live

Birth issued by the National Statistics Authority (NSO) and/or ALFREDO COBACHA
MERCADO, JR. as found in his Social Security System (SSS) Records, Senior Citizen
ID Card, Voter’s ID, Scholastics Records, Marriage Contract, Employment Records,
Pag-IBIG Records, and the Certificate of Birth of his child, Justine Kirsty L. Mercado,
POINT AND REFER TO ONLY ONE AND THE SAME PERSON;

We are disinterested in the purpose for which this affidavit is executed and
we are not related either by affinity or consanguinity within the fourth civil degree
of the above-named person and that we have no pecuniary interest in the execution
of this affidavit.

We are executing this affidavit to attest the foregoing facts for whatever legal
intents and purposes this may serve.

IN WITNESS WHEREOF, we have hereunto set our signatures this 18th day of
July 2018 in Palompon, Leyte, Philippines.
