To assist you in identifying the leadership competencies you would include in your Individual
Leadership Development Plan, please complete the section of the questionnaire based on the
Leadership Level you are pursuing.
1. Rate your proficiency for each competency using the following Competency Rating Scale.
[0] I have no opportunity to demonstrate competency
[1] I rarely demonstrate this competency (9-19%)
[2] I sometimes demonstrate this competency (20-39%)
[3] I often demonstrate this competency (40-59%)
[4] I consistency demonstrate this competency (60-89%)
[5] I always demonstrate this competency (>90%)
2. Calculate the Average Rating for each Competency in the Leadership Level.

3. Provide examples of how you have demonstrated the competency based on the proficiency rated you provided for
each.
4. Transfer the ratings of your Leadership Competency Self-Assessment to the applicable Outcome page in order to
identify strengths and areas of development to be shared/discussed with your Supervisor/Manager in applying for the
Emerging Leaders Program and subsequently designing your Individual Learning & Development Plan.
