 INSTRUCTIONS TO FILL THE REGISTRATION FORM
1. Send your Registration Form along with Original deposit slip in Pre paid Pakistan Postal Service Envelop.
2. Attach 2 recent photos of size (1x1.5 inch only) at the front of this Registration Form.
3. Deposit Rs: 300/- (Non Refundable/ Non Transferable) in Designated Bank branches of ABL, UBL or HBL, (Blank
Deposit Slip is attached with this Registration Form) and attach Original Deposit slip with your Registration form.
4. In column of Academic record must write your major subjects against every Certificate/ Degree in brief. Write down
percentage of secured marks in 2 digits only, forms having any information other than percentage will not be
processed.
5. Write two digit Test City Code for the desired Test Center choice on Para 2. of the Registration Form (look in
the chart below for Test City Codes).
