DIVISION (SSCD)

I. Frontline Service : APPLICATION FOR RADIO OPERATORS EXAMINATION

Clients : Seafarers/Aircraft Pilots/Radio Amateurs/Commercial Radio Operators

Requirements:

I. GOC
1. Certificate of Completion of GOC for GMDSS Training
2. Copy of birth certificate or any government issued ID indicating the birth date of the
applicant, i.e. passport, seaman’s book, driver’s license, PRC, GSIS, SSS, and others. (to
show original)
3. 2 pieces 1” X 1” recent ID picture (taken within the last 6 months)
4. 2 pieces mailing envelopes with stamps (size: 4 1/8” x 9 ½”)
5. *Copy of Certificate of Completion of Refresher Course

Note: *For those who were issued a training certificate under the 1978 STCW Convention with
no accumulated sea service performing the functions appropriate to the certificate held for at
least one (1) year in total for the preceding five (5) years.

b. Single Proprietor – Registration of Business issued by DTI
5. Copy of Certificate of Ownership of lot and building or Contract of Lease
6. Valid Mayor’s / Business Permit.
7. Copy of Proposed Course Curriculum
8. Copy of Class Programs
9. List of Instructors and corresponding “Certificates of Accreditation as Instructor” issued
by the Maritime Industry Authority (MARINA)
10. Copy of Teaching Loads of Instructors
11. List of laboratory and test equipment
12. List of textbooks and magazines
13. Copy of financial statement or proof of financial capability
14. Copy of economic and financial feasibility study
15. Clearance and/or Endorsement from the MARINA regarding the application and/or approval
of the offering of Short-term GMDSS radio communications courses.

NOTE: Certificate of Accreditation shall be issued after the training facilities have been inspected and
administrative requirements properly complied with.

PERMIT TO PURCHASE
1. Application for Permit to Purchase/Possess radio communications equipment
After purchase, applicant shall file for construction permit and radio station license.

NEW RADIO STATION LICENSE


1. Duly accomplished application form for Radio Station License (New)
2. Original copy of valid Permit to Purchase
3. Applicant shall submit proof of purchase from duly accredited radio dealers (if purchased
locally) or proof of payment of customs duties and clearance (if purchased abroad).
4. Copy of details of radio equipment’s make/type/ model and serial number/s
5. Antenna layout showing type of antenna; height; gain; type of mast; etc. signed and sealed by
a registered Professional Electronics Engineer (PECE)
6. Copy of valid radio operator’s certificate (GOC)
7. Certificate of employment of radio operator (GOC)

RENEWAL

3. Duly certified true copy of the Securities and Exchange Commission (SEC) Registration
Certificate and Articles of Incorporation for Corporation/Partnership or Department of Trade
and Industry (DTI) Registration Certificate for sole proprietor.
4. Ownership
a. Corporation – Copy of Articles of Incorporation issued by the SEC
i. 100% Filipino owned
ii. 60% Filipino, 40% Foreign (if with foreign partner)
b. Single Proprietor – Registration of Business issued by DTI
5. Copy of Certificate of Ownership of lot and building or Contract of Lease
6. Valid Mayor’s/Business Permit
7. Copy of financial feasibility and viability study, including proposed fees and charges.

Technical Requirements
1. Technical specifications of GMDSS simulator system including the description and functionalities
2. Floor plan of structural facilities (assessment room, briefing room, administrative room) duly
signed and sealed by a Professional Electronics Engineer (PECE)
3. Certificate of ownership or Lease agreement of the simulator equipment and maintenance contract
4. Copy of guidelines and procedures in the administration of simulator examination
5. Specimen signatures of authorized signatories
6. List of assessors with Certificate of Employment (please include Certificate of Training relevant
to the operation of the simulator equipment)
7. Affidavit of undertaking signed by the President/Owner/Authorized Representative stating in
substance and in principle that the MAC:

a. Shall ensure the integrity and honesty of the personnel and officials involved in the
conduct of assessment and shall adhere to the fair practice in conducting assessment with
the ultimate aim of producing highly competitive seafarers;
b. Shall strictly adhere and comply with the herein guidelines and that any violation hereof
shall be a ground for revocation, cancellation or suspension of the CA;
c. Shall be amenable to unscheduled/unannounced inspection; and
d. Management and its employees are not related by affinity and consanguinity up to the 1 st
degree with the officers/owners/board of directors of existing RMTIs.

Society and properly signed by any authorized representative of Philippine Consulate at the place
of survey (for vessel acquired under P.D. #866/1771 – International Trade).
4. Certificate of Inclusion from an Accounting Authority.
5. Copy of Certificate of Philippine Registry (CPR) issued by the Maritime Industry Authority
(MARINA).
6. Copy of valid Shore-Based Maintenance Agreement
7. Valid General Operators Certificates of two (2) Deck officers.
8. Contract of Employment of Deck officers issued by POEA/Certificate of Employment issued by
Shipping Company.

SURVEY FORM

2 At the end of the day, the CWPD shall collect all Concerned Unit
survey forms and transmit the same to the
Office Statistician designated to conduct a
statistical analysis of the survey.

3 A monthly summary of said statistical analysis Concerned Unit


shall be prepared by the Office Statistician who
conducted the study and forward the same to
the QMS leader to be submitted every
Wednesday of the succeeding week.
4 The QMS Leader, through the QMS Secretariat, Concerned Unit
shall determine if the feedback is meritorious
or not. If the feedback is meritorious, the QMS
Secretariat shall endorse the same to the
Commissioner and to the concerned
division/unit. Otherwise, the same shall be
stored as record on file.

5 The Chief of the concerned division/unit shall Concerned Unit


propose corrective action/s and discuss the
same with the QMS Leader.

6 The Chief of the concerned division/unit and Concerned Unit


the QMS Leader shall then make
recommendation on corrective action/s to take
and seek approval thereof by the
Commissioner.

7 If approved, the Chief of the concerned Concerned Unit


division/unit shall implement the corrective
action/s and report the result thereof to the
QMS Leader copy furnished the Office of the
Commissioner.

8 The Documentation Team shall be the Concerned Unit


repository of all documents, records and files
relevant hereto.
