 III. PRESENTING PROBLEM

Client, Rocky Boy, is asking to avail an Emergency Philhealth (PHIC) in behalf of his live-in
partner who was admitted at Howard Hubbard Hospital due to caesarian operation.

IV. BACKGROUND INFORMATION/ HISTORY

Client - Rocky Boy is 25 years old, eldest child of Mr. and Mrs. Brino Dizon. He works as

a construction worker earning an estimated monthly income of P3,500. While

her live in partner are jobless and just depending on the income of Rocky Boy,
which is insufficient to support, their basic needs of the family considering that
the source of income is irregular.

Family - Rocky Boy and his live in partner Analiza is living together for almost 1 year in

the parents’ house of Rocky Boy. On the year 2011, his mother and his sister died
due to an accident, while his father got married to other woman. Rocky Boy
is the only one who support the needs of his 2 siblings namely Michael who is
deaf and mute and out of school youth while Dominic is studying in Grade 6.
Despite that, they have no parent who is living with them. They still show’s love
and care to each other; they have a good relationship to each other.

Community- Rocky Boy is presently living at Purok 07 Daanbanwang, Upper Labay, G.S.C.

Where most of the residents living there are used to work as farmers for their
source of income. Residents were living near the river and free from bad
influence to the children. The Community has a very simple living and shows
respect and maintaining good relationships to each other.
V. ASSESSMENT

Client is seeking for Emergency Phil health, client is presently in distressed situation
due to difficulties in meeting the medical need of her live in partner. Client is
financially incapable to support her medication because his monthly income is not
enough to support the basic needs of the family.
Client must be given an immediate Intervention because if it’s not it might cause
dysfunctionality to the family.

VI. INTERVENTION PLAN

OVERALL GOAL: To assist client to avail emergency PHIC for their medical needs.
OBJECTIVES: To be able to apply for FRAME membership and avail benefits of PHIC.

VII. RECOMMENDATION

Based on the assessment and data gathered by the undersigned, Rocky T. Dizon
is recommended to be included as one of the beneficiaries Emergency Phil health
(PHIC) and availed the emergency medical services at any Hospital within the
Philippines for the possible immediate medical assistance.
