SWORN STATEMENT

WEST TIMAWA, MOLO, ILOILO CITY
Married, Filipino and with residence address at, ____________________________________,
after having been duly sworn to in accordance with law, do hereby depose and say:

1. That in behalf of West Timawa People’s Organization, I am applying for a preliminary

approval/ locational clearance before the Committee on Urban Planning, Expropriation,
Assessment Housing and Land Development of the Iloilo City Council;
2. That in accordance with BP 220 (Socialized Housing) we are required to submit a
statement as to the nature, number and income level of the qualified beneficiaries of the
Socialized Housing Project;
3. That based on the Community Profile provided by the Barangay West Timawa, West
Timawa People’s Organization is a non-stock corporation duly registered with the
Securities and Exchange Commission (SEC) last June 2, 2016. It is composed of 59
informal settlement families (ISFs), with an average income ranging from FIVE
THOUSAND to EIGHT THOUSAND PESOS, occupying a parcel of land with a total area
of 5,540 square meters owned by Mr. Ramon dela Cruz located at Brgy. West Timawa,
Molo, Iloilo City;
4. That in event that I made full and deliberate assertion of falsehood in the foregoing
statement I shall be liable for perjury punishable under Act. 183 of the Revised Penal
Code;
5. That furthermore, untruthful statement in narration of materials facts shall permanently bar
the issuance of the said clearance;
6. That I execute this affidavit in order to attest the truth of the above facts for all legal intents
and purpose it may serve.
