AFFIDAVIT OF EXPLANATION

I, MARY ANN M. MANAMAN, of legal age, Filipino, married, and a

resident of Brgy. Santo Anghel Central, Santa Cruz, Laguna Philippines, after
having been duly sworn in accordance with law, hereby depose and state:

That on 12 May 2011, I was issued a Philippine Passport by the Philippine

Embassy in Doha, Qatar;

That on 02 December 2015 the said passport was lost and deemed beyond
recovery after an Indian national named Firoz Pazhayakath took it away and never
returned, after we gave him money for renewal of our Qatar Visa;

That inspite of diligent search efforts to locate the said Indian national to
recover my passport, I could not find him anymore, such that I went to the
Philippine Embassy which issued to me a Travel Document so that I can return to
the Philippines;

I executed this Affidavit of Explanation to attest to the truth of the

foregoing and to support my application for the issuance of a new passport.

IN WITNESS WHEREFORE, I hereto affixed my signature this 17 September

2016 in Santa Cruz, Laguna.
