SUBJECT: Staff Memo dated April 25, 2013 regarding Proposed new Acceptance Criteria for
Headed Cast-in Specialty Inserts in Concrete, Subject AC446-0613-R2

Dear Andra:
We are writing to comment on the proposed new acceptance criteria for cast-in specialty inserts
in concrete, AC446. In response to the staff memo of April 25, 2013 and the resolution letter
from the February ICC-ES Committee hearings dated March 8, 2013, CAMA offers the attached
revision to the draft of AC446 dated February 2, 2013. We would also like to respond to the
points raised in Mr. Nichols’ letter as follows:
1. Comment: The need for additional information regarding symbols and terminology used in the
criteria. Response: The attached draft includes definitions of the terms and symbols used in the
criteria. (See Section 1.5 – Terminology)
2. Comment: The need for improved scope definition. Response: The attached draft provides
additional clarification regarding the scope of the criteria as pertaining to headed specialty
inserts. Additional figures have also been provided. (See Section 1.2 Scope and Figs. 1-1, 1-2,
and 1-3.)
3. Comment: The need for additional information regarding the manufacture of the insert.
Response: The attached draft contains requirements for reporting on the manufacture of specialty
inserts. (See Sections 2.1.1, 2.4.3, and 2.4.4.)
4. Comment: The need for better definition of geometrical limitations/restrictions on the insert,
and the values to be used in design. Response: The attached draft contains requirements on
definition of the geometrical requirements for specialty inserts. (See revised Section 3.4.2.)
5. Comment: The justification for omitting tension testing in concrete. Response: Tension testing
of headed studs and bolts is not required to design these anchor types in accordance with the
provisions of ACI 318 Appendix D. Similarly, if the bearing stress limitations, e.g., of ACI 318
D.5.3 have been met, there is no need to perform tension tests on headed specialty inserts to
validate the applicability of Appendix D.
6. Comment: The need for additional information regarding the applicability of the design
provisions of Appendix D of ACI 318 to inserts installed in the soffit of concrete over metal deck
assemblies. Response: Attached is a summary of tension tests performed on headed specialty
inserts installed in the soffit of concrete on metal deck assemblies in which concrete breakout
failure occurred. These tests indicate that the use of the Appendix D provisions as outlined in the
criteria provide a reasonable and conservative approximation of the actual behavior. The
attached draft contains an exception that permits testing to validate less conservative values. (See
Section 3.5.4.)
7. Whether compression testing of anchors installed in the upper flute of metal deck is necessary.
Response: In general, compression loads should be taken up by a nut and washer assembly at the
surface of the deck or the attachment (see Fig. 1-1 of the attached draft). Where this is not
possible, the concrete breakout capacity of a headed anchor in compression can be calculated
using the Appendix D provisions.
8. The need for a table of design data. Response: The attached draft includes an example table of
design data. (Table 6.1)
The attached draft also contains improved requirements for characterization of headed specialty
inserts as ductile or non-ductile for the purposes of design in accordance with ACI 318 Appendix
D, a statement on fatigue and shock loading, and a statement on special inspection requirements.
We will be available to respond to questions at the ICC-ES hearings in June.

PREFACE
Evaluation reports issued by ICC Evaluation Service, Inc. (ICC-ES), are based upon performance
features of the International family of codes and other widely adopted code families, including the Uniform
Codes, the BOCA National Codes, and the SBCCI Standard Codes. Section 104.11 of the International Building
®
Code reads as follows:
The provisions of this code are not intended to prevent the installation of any materials or to
prohibit any design or method of construction not specifically prescribed by this code,
provided that any such alternative has been approved. An alternative material, design or
method of construction shall be approved where the building official finds that the proposed
design is satisfactory and complies with the intent of the provisions of this code, and that the
material, method or work offered is, for the purpose intended, at least the equivalent of that
prescribed in this code in quality, strength, effectiveness, fire resistance, durability and safety.
Similar provisions are contained in the Uniform Codes, the National Codes, and the Standard Codes.
This acceptance criteria has been issued to provide all interested parties with guidelines for demonstrating
compliance with performance features of the applicable code(s) referenced in the acceptance criteria. The criteria
was developed and adopted following public hearings conducted by the ICC-ES Evaluation Committee, and is
effective on the date shown above. All reports issued or reissued on or after the effective date must comply with
this criteria, while reports issued prior to this date may be in compliance with this criteria or with the previous
edition. If the criteria is an updated version from the previous edition, a solid vertical line (|) in the margin within
the criteria indicates a technical change, addition, or deletion from the previous edition. A deletion indicator (→)
is provided in the margin where a paragraph has been deleted if the deletion involved a technical change. This
criteria may be further revised as the need dictates.
ICC-ES may consider alternate criteria, provided the report applicant submits valid data demonstrating that the
alternate criteria are at least equivalent to the criteria set forth in this document, and otherwise demonstrate
compliance with the performance features of the codes. Notwithstanding that a product, material, or type or
method of construction meets the requirements of the criteria set forth in this document, or that it can be
demonstrated that valid alternate criteria are equivalent to the criteria in this document and otherwise
demonstrate compliance with the performance features of the codes, ICC-ES retains the right to refuse to issue
or renew an evaluation report, if the product, material, or type or method of construction is such that either
unusual care with its installation or use must be exercised for satisfactory performance, or if malfunctioning is
apt to cause unreasonable property damage or personal injury or sickness relative to the benefits to be achieved
by the use of the product, material, or type or method of construction.
Acceptance criteria are developed for use solely by ICC-ES for purposes of issuing ICC-ES evaluation reports.

1.0 INTRODUCTION
1.1 Purpose: The purpose of this criteria is to establish requirements for ICC Evaluation Service, Inc. (ICC-ES),
recognition of headed cast-in specialty inserts in concrete under Sections 1911, 1912, and 2303.5 of the 2012, 2009
®
and 2006 International Building Code (IBC), and Section R104.11 of the 2012, 2009 and 2006 International
®
Residential Code (IRC).
1.2 Scope:
1.2.1 The provisions of this criteria shall be used to derive the capacity of headed specialty inserts cast in concrete
for purposes of transmitting tension and shear loads. Typical headed cast-in specialty inserts are shown in
Figures 1-1 and 1-2.
1.2.2 This criteria applies to headed specialty inserts that transfer tension loads to the concrete via a bearing head at
the embedded end of the anchor. It does not apply to coil anchors, hooked bolts, or other non-headed cast-in
anchor systems. See Fig. 1-3.
1.2.3 The provisions of this criteria shall be used to establish the strength of headed cast-in specialty inserts based
on the strength design provisions of ACI 318.
1.2.4 The provisions of this criteria shall be applicable only to headed cast-in specialty inserts as defined in Section
1.4 of this criteria.

2.1.1 Product Description: Complete information pertaining to the headed cast-in specialty insert, including material
specifications, drawn-to-scale production drawings showing all dimensions and tolerances, description of the
manufacturing process (including welds when applicable). Materials shall comply with recognized standards.
2.1.2 Manufacturer’s Printed Installation Instructions (MPII): Published instructions for correct anchor installation
under all covered installation conditions as supplied in product packaging by the manufacturer of the headed
cast-in specialty insert.
2.1.3 Packaging and Identification: A description of field identification of the insert. Each headed cast-in specialty
insert shall bear an imprint that clearly identifies the manufacturer (a registered trademark may serve as such
identity). Labeling shall also include the model number and the ICC-ES evaluation report number.
2.2 Testing Laboratories: Testing laboratories shall comply with Section 2.0 of the ICC-ES Acceptance Criteria for Test
Reports (AC85) and Section 4.2 of the ICC-ES Rules of Procedure for Evaluation Reports.
2.3 ITEA: The testing and evaluation of anchors under this criteria shall be performed or witnessed by an independent
testing and evaluation agency (ITEA) or agencies accredited under ISO/IEC 17025 by a recognized accreditation
body conforming to the requirements of ISO/IEC 17011. In addition to these standards, listing of the ITEA shall be
predicated on documented experience in the testing and evaluation of anchors according to ASTM E488, including
demonstrated competence to perform the tests described in this criteria. The ITEA shall verify that all elements of
the test program and analysis are in compliance with this criteria and shall conduct or directly verify all procedures.
2.4 Test Reports: Test reports shall comply with AC85 and include the following information:
2.4.1 Identification of the test standard used and the date of issue of the standard, and other relevant information
concerning the test procedure, justification for any deviations from the referenced test standard, and any critical
information relevant to the specific test.
2.4.2 A description of the sample size of randomly selected headed cast-in specialty inserts for each test protocol.
2.4.3 Production drawings of the tested headed cast-in specialty insert, providing bolt dimensions and identifying bolt
steel specification(s).
2.4.4 Specified and measured physical properties of the headed cast-in specialty insert.
2.4.5 Photographs of the test setup and typical failure modes shall be included in the test report.
2.4.6 Location of displacement instrumentation and their point of reference as well as load-versus-deformation
curves, as plotted directly, or as reprinted from data acquisition systems.
2.4.7 Individual and average ultimate test load values with standard deviation and a description of the general
behavior of tested headed cast-in specialty inserts during load application; and a description of the nature, type
and location of failures of tested headed cast-in specialty inserts and bolts.

2.4.8 Description of the test setup and the cyclic protocol when cyclic testing is performed in accordance with Section
3.5.4 and 3.6.3 of this criteria.
2.5 Product Sampling: Sampling shall comply with Section 3.2 of AC85.

3.2.1 The concrete used in the anchor tests of Section 3.5 and 3.6 shall comply with the following:
3.2.1.1 Concrete mix design shall follow recommendations for proportioning in ACI 211.1 or ACI 211.2 or ACI
318. The concrete shall also comply with Section 5.1 of ACI 355.2. Proportions may be varied to meet
local requirements and to achieve desired nominal compressive strength. The reason for any variation

shall be described in the test report. No cementitious additives shall be added to the concrete test
members.
3.2.1.2 Portland cement shall comply with ASTM C 150. Additional requirements in Section 5.1.2 of ACI 355.2
apply.
3.2.1.3 Coarse and fine aggregate in concrete shall comply with ASTM C 33 for normal-weight concrete and
ASTM C 330 for structural lightweight concrete. The aggregate description shall include the rock and
mineral components, shape, hardness, maximum size, and grading specification. Additional
requirements in Section 5.1.1 of ACI 355.2 also apply.
3.2.1.4 Concrete test cylinders shall be prepared in accordance with ASTM C 31. Cylinders shall be stored and
cured in accordance with Section 9.3.1 of ASTM C 31 (field cure). To determine the compressive
strength of test members, cylinders shall be tested in accordance with ASTM C 39 and Section 3.2.8 of
this criteria.
3.2.1.5 When no test cylinders are available, or where there is a question as to whether the concrete cylinders
represent the concrete compressive strength of the test members, compressive strength shall be
determined using drilled cores from test members. Cores shall be obtained, prepared, and tested in
accordance with ASTM C 42 and Section 3.2.8 of this criteria.
3.2.2.1 Concrete compressive strengths at time of anchor tests shall comply with the ranges specified in Section
5.1.3 of ACI 355.2.
3.2.2.2 Test members shall be cured a minimum of 21 days prior to the beginning of anchor tests in accordance
with Section A3.3 of ACI 355.2.
3.2.2.3 Where high-early-strength cement is used, curing shall be for a minimum of seven days, with a two-day
allowable minus tolerance. Two cylinders shall be tested in accordance with ASTM C 39 to determine
compressive strength. The average compressive strength shall be determined within12-hour period
immediately preceding or following any test series.
3.2.2.4 For concrete less than 90 days old, two tests of two cylinders, or cores, each prepared according to
Section 3.2.1.4 of 3.2.1.5, shall be performed at the beginning and ending of anchor testing in
accordance with Table 1. The beginning of the anchor test shall be concurrent with the initiation of
anchor testing. The beginning and ending strength results shall be averaged (four cylinders or cores,
total) to establish the strength of the test members during the test period.
3.2.2.5 For concrete aged 90 days or more, the compressive strength test shall be a single test of three
cylinders or cores, performed within 30 days of any anchor testing.
3.2.2.6 Reported concrete compressive strength for any anchor test series shall be determined from tests in this
section, within the time limitations shown in Table 2 of this criteria.
3.2.3.1 Bolt: Determine bolt tensile strength in accordance with ASTM F 606. Bolts shall comply with
ANSI/ASME Standard B18.2.1. All steel properties of the bolt or threaded rod, including tensile strength

and measured diameter shall be determined. This data is permitted to be obtained from the mill
certification of the steel from which the insert is manufactured. For characterization of headed cast-in
specialty insert as ductile, see Section 3.8.
3.2.3.2 Head and shaft: Provide material properties for the insert head and shaft.
3.2.4 Other materials: Where other materials (e.g., plastic) are used in the manufacture of the insert, these shall be
described in accordance with applicable standards.
3.2.5 Durability: When anchors are recognized for exterior exposure or damp environments, evidence of durability
shall be submitted. The steel shall be corrosion-resistant, stainless, or zinc-coated steel. The zinc coating shall
be either hot-dipped in accordance with ASTM A 153 Class C or D; mechanically deposited in accordance with
ASTM B 695 with a Class 55 coating having a minimum thickness of 2.1 mils (0.053 mm); or demonstrated
through tests to be equivalent to the coatings previously described in this sentence. When durability
specifications (corrosion-resistant coatings or materials) are given by the manufacturer, the anchors shall be
tested for conformance to the specified standards.
3.3.1 General: Test members shall be prepared in accordance with Section 6.4 of ASTM E 488 and Appendix A3 of
ACI 355.2.
3.3.2 Concrete-filled Metal Deck: Concrete used as fill on metal deck test specimens shall be prepared as follows:
3.3.2.1 Cast test members having the minimum concrete fill thickness, maximum decking depth, minimum flute
width and minimum metal thickness that the assessment should cover. The concrete fill shall contain no
reinforcing. Lightweight concrete fill shall be permitted to be used. Test members may be inverted and
placed flat on the floor of the test facility to facilitate shear testing of anchors installed through the
decking and into the fill concrete.
3.3.2.2 Concrete fill on metal decking specimens shall represent the minimum concrete fill thickness, maximum
decking depth, minimum flute width and minimum decking thickness for this application. Results for tests
conducted in structural lightweight concrete fill shall be permitted to be used for applications in normal-
weight concrete fill. Metal decks used in all tests shall be evaluated by material property tests to
determine the tensile strength, yield strength, and elongation in accordance with the appropriate
standard for the metal grade. In addition, the base-metal thickness shall be determined, exclusive of
coatings. Test results shall be based on the evaluation of three specimens in each thickness. Where
measured properties vary from specified values, the influence of the variations on anchor tests shall be
considered in the analysis.
3.3.3 Reinforcement may only be used to stabilize test members during transportation or as required in Sections
A3.1.2 and A3.2 of ACI 355.2. Reinforcing elements in concrete test members shall be outside the potential
failure region of each test specimen or anchor group. The test laboratory shall verify the location of the
reinforcing.

3.8.1 For characterization of the headed cast-in specialty insert as ductile in accordance with ACI 318, conduct
tension tests on the specialty insert in accordance with Section 4.1. Characterize failure of the insert in
accordance with Table 3.1.
If these requirements are not met, the specialty insert shall be characterized as brittle. Assignment of strength
reduction factors in accordance with ACI 318 Appendix D 4.3 shall be made in accordance with Table 3.2 and
reported on the data sheet.
Exception: For internally-threaded inserts, perform tests with the insert equipped with a bolt or threaded rod of
sufficient strength to result in tension failure of the insert. It shall be permitted to characterize the failure of
internally-threaded inserts solely on the basis of evaluation in accordance with line no. 3 of Table 3.3.
3.8.2 For characterization of bolts or threaded rods to be used with the headed cast-in specialty insert as ductile in
accordance with ACI 318, tension tests on the bolt or rod shall be conducted in accordance with Table 3.2. If
these requirements are not met, the specialty insert shall be characterized as brittle. Assignment of strength
reduction factors in accordance with ACI 318 Appendix D 4.3 shall be made in accordance with Table 3.3 and
reported on the data sheet.

6.4 Relevant dimensional information for the use of headed cast-in specialty inserts in concrete on metal deck
assemblies as well as the supplemental conditions for design in accordance with Fig. 3-3 shall be provided.
6.5 The evaluation report shall state that structural design of anchors shall be performed in accordance with ACI 318
Appendix D.
Strength reduction factors shall be in accordance with the provisions of ACI 318 as applicable for cast-in bolts.
Determination of tension strength as governed by concrete breakout or side-face blowout shall be in accordance with
the provisions of ACI 318 as applicable for cast-in bolts.
Requirements on anchor edge distance, anchor spacing, member thickness, and concrete strength shall be in
accordance with the requirements ACI 318 as applicable for cast-in bolts.
Periodic special inspection is required in accordance with Section 1705.1.1 of the 2012 IBC, Section 1704.15 of the
2009 IBC, and Section 1704.13 of the 2006 IBC. For each type of anchoring system, the manufacturer shall submit
inspection procedures to verify proper usage.

Fire-resistive Construction: Where not otherwise prohibited in the code, anchors are permitted for use with fire-
resistance rated construction provided that at least one of the following conditions is fulfilled:
7.0 Fatigue and shock loading: Since an ICC-ES acceptance criteria for evaluating data to determine the performance of
anchors subjected to fatigue or shock loading is unavailable at this time, the use of these anchors under these conditions
is beyond the scope of this report.
8.0 Special inspection: Special inspection details based on information described in Section 5.4 shall be included in the
evaluation report. The special inspector shall make periodic inspections during anchor installation to verify anchor type,
anchor dimensions, concrete type, concrete compressive strength, hole dimensions, anchor spacing, edge distances,
concrete thickness, anchor embedment, and adherence to the manufacturer's published installation instructions. The
special inspector shall be present as often as required in accordance with the “statement of special inspection.”

PREFACE
Evaluation reports issued by ICC Evaluation Service, Inc. (ICC-ES), are based upon performance
features of the International family of codes and other widely adopted code families, including the Uniform
Codes, the BOCA National Codes, and the SBCCI Standard Codes. Section 104.11 of the International Building
®
Code reads as follows:
The provisions of this code are not intended to prevent the installation of any materials or to
prohibit any design or method of construction not specifically prescribed by this code,
provided that any such alternative has been approved. An alternative material, design or
method of construction shall be approved where the building official finds that the proposed
design is satisfactory and complies with the intent of the provisions of this code, and that the
material, method or work offered is, for the purpose intended, at least the equivalent of that
prescribed in this code in quality, strength, effectiveness, fire resistance, durability and safety.
Similar provisions are contained in the Uniform Codes, the National Codes, and the Standard Codes.
This acceptance criteria has been issued to provide all interested parties with guidelines for demonstrating
compliance with performance features of the applicable code(s) referenced in the acceptance criteria. The criteria
was developed and adopted following public hearings conducted by the ICC-ES Evaluation Committee, and is
effective on the date shown above. All reports issued or reissued on or after the effective date must comply with
this criteria, while reports issued prior to this date may be in compliance with this criteria or with the previous
edition. If the criteria is an updated version from the previous edition, a solid vertical line (|) in the margin within
the criteria indicates a technical change, addition, or deletion from the previous edition. A deletion indicator (→)
is provided in the margin where a paragraph has been deleted if the deletion involved a technical change. This
criteria may be further revised as the need dictates.
ICC-ES may consider alternate criteria, provided the report applicant submits valid data demonstrating that the
alternate criteria are at least equivalent to the criteria set forth in this document, and otherwise demonstrate
compliance with the performance features of the codes. Notwithstanding that a product, material, or type or
method of construction meets the requirements of the criteria set forth in this document, or that it can be
demonstrated that valid alternate criteria are equivalent to the criteria in this document and otherwise
demonstrate compliance with the performance features of the codes, ICC-ES retains the right to refuse to issue

1.0 INTRODUCTION
1.1 Purpose: The purpose of this criteria is to establish requirements for ICC Evaluation Service, Inc. (ICC-ES),
recognition of headed cast-in specialty inserts in concrete under Sections 1911, 1912, and 2303.5 of the 2012, 2009
and 2006 International Building Code® (IBC), and Section R104.11 of the 2012, 2009 and 2006 International
®
Residential Code (IRC).
1.2 Scope:
1.2.1 The provisions of this criteria shall be used to derive the capacity of headed specialty inserts cast in concrete
for purposes of transmitting tension and shear loads. Typical installation headed cast-in specialty
insertsexamples are shown in Figures 1-1 and 1-2.
1.2.2 This criteria applies to headed specialty inserts that transfer tension loads to the concrete via a bearing head at
the embedded end of the anchor. It does not apply to coil anchors, hooked bolts, or other non-headed cast-in
anchor systems. See Fig. 1-32.
1.2.3 The provisions of this criteria shall be used to establish the strength of specialty insertheaded cast-in specialty
inserts based on the strength design provisions of ACI 318.
1.2.4 The provisions of this criteria shall be applicable only to specialty insertheaded cast-in specialty inserts as
defined in Section 1.4 of this criteria.

2.1.1 Product Description: Complete information pertaining to the specialty insertheaded cast-in specialty insert,
including material specifications, drawn-to-scale production drawings showing all dimensions and tolerances,
description of the manufacturing process (including welds when applicable). Materials shall comply with
recognized standards.

2.2 Testing Laboratories: Testing laboratories shall comply with Section 2.0 of the ICC-ES Acceptance Criteria for Test
Reports (AC85) and Section 4.2 of the ICC-ES Rules of Procedure for Evaluation Reports.

2.3 ITEA: The testing and evaluation of anchors under this criteria shall be performed or witnessed by an independent
testing and evaluation agency (ITEA) or agencies accredited under ISO/IEC 17025 by a recognized accreditation
body conforming to the requirements of ISO/IEC 17011. In addition to these standards, listing of the ITEA shall be
predicated on documented experience in the testing and evaluation of anchors according to ASTM E488, including
demonstrated competence to perform the tests described in this criteria. The ITEA shall verify that all elements of
the test program and analysis are in compliance with this criteria and shall conduct or directly verify all procedures.
2.4 Test Reports: Test reports shall comply with AC85 and include the following information:
2.4.1 Identification of the test standard used and the date of issue of the standard, and other relevant information
concerning the test procedure, justification for any deviations from the referenced test standard, and any critical
information relevant to the specific test.
2.4.2 A description of the sample size of randomly selected specialty insertheaded cast-in specialty inserts for each
test protocol.
2.4.3 Production drawings of the tested specialty insertheaded cast-in specialty insert, providing bolt dimensions and
identifying bolt steel specification(s).
2.4.4 Specified and measured physical properties of the specialty insertheaded cast-in specialty insert.
2.4.5 Photographs of the test setup and typical failure modes shall be included in the test report.
2.4.6 Location of displacement instrumentation and their point of reference as well as load-versus-deformation
curves, as plotted directly, or as reprinted from data acquisition systems.
2.4.7 Individual and average ultimate test load values with standard deviation and a description of the general
behavior of tested specialty insertheaded cast-in specialty inserts during load application; and a description of
the nature, type and location of failures of tested specialty insertheaded cast-in specialty inserts and bolts.
2.4.8 Description of the test setup and the cyclic protocol when cyclic testing is performed in accordance with Section
3.5.4 and 3.6.3 of this criteria.
2.5 Product Sampling: Sampling shall comply with Section 3.2 of AC85.

3.2.1 The concrete used in the anchor tests of Section 3.5 and 3.6 shall comply with the following:
3.2.1.1 Concrete mix design shall follow recommendations for proportioning in ACI 211.1 or ACI 211.2 or ACI
318. The concrete shall also comply with Section 5.1 of ACI 355.2. Proportions may be varied to meet
local requirements and to achieve desired nominal compressive strength. The reason for any variation
shall be described in the test report. No cementitious additives shall be added to the concrete test
members.
3.2.1.2 Portland cement shall comply with ASTM C 150. Additional requirements in Section 5.1.2 of ACI 355.2
apply.
3.2.1.3 Coarse and fine aggregate in concrete shall comply with ASTM C 33 for normal-weight concrete and
ASTM C 330 for structural lightweight concrete. The aggregate description shall include the rock and
mineral components, shape, hardness, maximum size, and grading specification. Additional
requirements in Section 5.1.1 of ACI 355.2 also apply.
3.2.1.4 Concrete test cylinders shall be prepared in accordance with ASTM C 31. Cylinders shall be stored and
cured in accordance with Section 9.3.1 of ASTM C 31 (field cure). To determine the compressive
strength of test members, cylinders shall be tested in accordance with ASTM C 39 and Section 3.2.8 of
this criteria.

3.2.1.5 When no test cylinders are available, or where there is a question as to whether the concrete cylinders
represent the concrete compressive strength of the test members, compressive strength shall be
determined using drilled cores from test members. Cores shall be obtained, prepared, and tested in
accordance with ASTM C 42 and Section 3.2.8 of this criteria.
3.2.2.1 Concrete compressive strengths at time of anchor tests shall comply with the ranges specified in Section
5.1.3 of ACI 355.2.
3.2.2.2 Test members shall be cured a minimum of 21 days prior to the beginning of anchor tests in accordance
with Section A3.3 of ACI 355.2.
3.2.2.3 Where high-early-strength cement is used, curing shall be for a minimum of seven days, with a two-day
allowable minus tolerance. Two cylinders shall be tested in accordance with ASTM C 39 to determine
compressive strength. The average compressive strength shall be determined within12-hour period
immediately preceding or following any test series.
3.2.2.4 For concrete less than 90 days old, two tests of two cylinders, or cores, each prepared according to
Section 3.2.1.4 of 3.2.1.5, shall be performed at the beginning and ending of anchor testing in
accordance with Table 1. The beginning of the anchor test shall be concurrent with the initiation of
anchor testing. The beginning and ending strength results shall be averaged (four cylinders or cores,
total) to establish the strength of the test members during the test period.
3.2.2.5 For concrete aged 90 days or more, the compressive strength test shall be a single test of three
cylinders or cores, performed within 30 days of any anchor testing.
3.2.2.6 Reported concrete compressive strength for any anchor test series shall be determined from tests in this
section, within the time limitations shown in Table 2 of this criteria.
3.2.3.1 Bolt: Determine bolt tensile strength in accordance with ASTM F 606. Bolts shall comply with
ANSI/ASME Standard B18.2.1. All steel properties of the bolt or threaded rod, including tensile strength
and measured diameter shall be determined. This data is permitted to be obtained from the mill
certification of the steel from which the insert is manufactured. For characterization of specialty
insertheaded cast-in specialty insert as ductile, see Section 3.8.
3.2.3.2 Head and shaft: Provide material properties for the insert head and shaft.
3.2.4 Other materials: Where other materials (e.g., plastic) are used in the manufacture of the insert, these shall be
described in accordance with applicable standards.
3.2.5 Durability: When anchors are recognized for exterior exposure or damp environments, evidence of durability
shall be submitted. The steel shall be corrosion-resistant, stainless, or zinc-coated steel. The zinc coating shall
be either hot-dipped in accordance with ASTM A 153 Class C or D; mechanically deposited in accordance with
ASTM B 695 with a Class 55 coating having a minimum thickness of 2.1 mils (0.053 mm); or demonstrated
through tests to be equivalent to the coatings previously described in this sentence. When durability

effect of flexural stresses on anchor and member behavior. The results of tests shall be applicable to the
specific geometry and material parameters tested.
3.5.43.5.5 Simulated seismic tension tests: For recognition of anchors for use in Seismic Design Category
C, D, E or F, perform simulated seismic tension tests on anchors mounted in a jig as shown in Figure 4-1. The

3.83.8.2 For characterization of bolts or threaded rods to be used with the headed cast-in specialty insert as ductile in

6.1 The evaluation report shall describe the insert with respect to material specifications.
6.2 The evaluation report shall provide a table (see Table 6.1) or table footnotes specifying the following information:

6.4 Relevant dimensional information for the use of headed cast-in specialty inserts in concrete on metal deck
assemblies as well as the supplemental conditions for design in accordance with Fig. 3-3 shall be provided.
6.36.5 The evaluation report shall state that structural design of anchors shall be performed in accordance with ACI
318 Appendix D.
Strength reduction factors shall be in accordance with the provisions of ACI 318 as applicable for cast-in bolts.
Determination of tension strength as governed by concrete breakout or side-face blowout shall be in accordance with
the provisions of ACI 318 as applicable for cast-in bolts.
Requirements on anchor edge distance, anchor spacing, member thickness, and concrete strength shall be in
accordance with the requirements ACI 318 as applicable for cast-in bolts.
Periodic special inspection is required in accordance with Section 1705.1.1 of the 2012 IBC, Section 1704.15 of the
2009 IBC, and Section 1704.13 of the 2006 IBC. For each type of anchoring system, the manufacturer shall submit
inspection procedures to verify proper usage.
Fire-resistive Construction: Where not otherwise prohibited in the code, anchors are permitted for use with fire-
resistance rated construction provided that at least one of the following conditions is fulfilled:

7.0 Fatigue and shock loading: Since an ICC-ES acceptance criteria for evaluating data to determine the performance of
anchors subjected to fatigue or shock loading is unavailable at this time, the use of these anchors under these conditions
is beyond the scope of this report.
8.0 Special inspection: Special inspection details based on information described in Section 5.4 shall be included in the
evaluation report. The special inspector shall make periodic inspections during anchor installation to verify anchor type,
anchor dimensions, concrete type, concrete compressive strength, hole dimensions, anchor spacing, edge distances,
concrete thickness, anchor embedment, and adherence to the manufacturer's published installation instructions. The
special inspector shall be present as often as required in accordance with the “statement of special inspection.”

Dear Andra:
As you are aware, the Concrete Anchor Manufacturers Association (CAMA) has been engaged in ongoing
discussions regarding the subject criteria and will be issuing a revised draft for the consideration of staff and the ICC-
ES committee at the June ICC-ES Committee hearing. I therefore propose to retract my letter of March 10, 2013 (and
the revisions to AC446 indicated in the attachment to that letter) in deference to the CAMA submission.
Having reviewed the proposed AC446 revisions of February, 13, 2013, we offer the following comments
many of which carry over from ISAT’s original comments dated, 01/18/13 in advance of the Birmingham
meetings.
ISAT Comments:
Proposed Acceptance Criteria AC446 for Cast-In Specialty Inserts
1. Sec. 3.6.3 Simulated Seismic Shear Tests: Cyclic seismic shear loading in the low flute is not a
requirement of AC-193 (June, 2012) for post installed anchors. As a result we see no need for this
requirement in AC446. The ICC-ES Memo of 12/1/2010, on this topic provides sufficient guidance.
2. It is imperative that results from tension testing in concrete be allowed as an alternate to AC446, Table
4.1, Test No. 2.
General Format
• Provide an index.
• Provide a nomenclature section that defines the variables used in one place.
• Expand the definition section to include things like “characteristic value”, “effective embedment
depth”, “Independent Testing and Evaluation Agency”.
• Remember this document is written for a wider audience that are not experts in the field of concrete
anchors.
• How does one know that the design procedures in ACI are valid? By testing and evaluation of the
results.
• Provide an alternate qualifier to Eq. 3.3 allowing higher values if substantiated by reliable cyclic testing
in cracked concrete.
• There is no need for an increase in steel strength for deeper embedment when the design value is
limited by concrete breakout strength.
• Example: 4” embedment with a headed insert commonly available today would require a head
thickness of 5/16”. It is hard to imagine that concrete bearing stress on steel cantilever of
approximately 5/8” would require a steel thickness of 5/16”.
• 3.5.3 Determination of concrete breakout strength in metal deck using ACI equations is questionable
and needs to be verified by testing. The insert takes up a large volume of concrete in this limited
space.
• 3.5.3 Nbasic should be substantiated by testing. ISAT documented testing showed a failure pattern in
low flutes on metal deck different from the proposed analytical approach.
Since composite deck provides some degree of confinement and reinforcement, it is not
possible to neglect the influence of the metal deck even before the concrete fails.
How can the strength of the metal deck be deducted since it is a “composite” section? By
what means is this to be accomplished? Are we to take the peak value (if so – what is the
difference); some percentage of the peak value?; or some other method? This sentence needs
to be removed or expanded to provide specific guidance as to how the “metal deck is to be
neglected”. Is the concrete assumed to cracked or un-cracked? How is a concrete slab on
metal deck to be evaluated for the cracked and un-cracked conditions?
• Why would a full series of tests be required for solid light weight concrete when ACI 318 has defined a
reduction factor for light weight concrete?
