Plant : U6500
Item : F6502 (HRSG-2) Burner Tips
Subject : 2 Burner Tips with Damaged Threads

Background and Findings:

• HRSG-2 was decommissioned for annual statutory inspection.


• As part of workscope, the burner tips were removed for cleaning. However, 2 burner tips at the topmost header
cannot be removed. The burner tips were just rotating around its female mating coupling which indicated that the
threads were already damaged.
• These two tips are tip number 5 and 11. Counting starts from west to east.

Recommendation(s):

See attached photo for reference

1. Forcefully pull the burner tips using pulling tool (pliers, etc.). Heat the burner tips, as necessary.
2. Restore the damaged threads on the female branch couplings using re-tapping tool.
3. If Step 2 is not successful, weld buildup the female branch coupling using E310 (SMAW) or ER310
(GTAW). Perform PMI of welding electrode or filler rod.
4. Restore the damaged threads on the female branch couplings using re-tapping tool. Perform DPT and
Visual Inspection.
5. Re-insert new burner tip or the old burner tip (if threads of the tip are still good).

Note:
Maintenance Team may remove the deflector plate if access is limited. The deflector plate holder pin should be cut
and then re-welded using E310 (SMAW) or ER310 (GTAW).
