Dear Sirs,

Appoint Mr. demand, the PERI FORMWORK for the AUSTRALIAN EMBASSY PROJECT at Jakarta, then
together we apply the following rates :
SPECIFICATIONS : - (Attached – 2 Summary sheets, 16 Estimates sheets, 15 Picture sheets )
Type of Goods

PRICE OF GOODS : - (Attached)

DELIVERY OF GOODS : - Delivery of Goods / Equipment rentalbooking:
i. BPW in the warehouse, where the customer has to check the
numberand quality and delivery is gradually.
ii. After all the customer approved the contract documents.
iii. All the technical details agreed.
iv. All contractual obligations fulfilled by the customer, including the down
payment / security / first payment.
TERMS OF OFFER : - Not including Taxes.
- Loco Warehouse PT. BETON PERKASA WIJAKSANA (BPW),in Cikupa-
Tangerang.
- Valid for 30 days.
- Not including timber, plywood, PVC pipe, sleeper, lifting equipmentand workers.
TERMS OF PAYMENT :
- SELL - First Payment / Advance / Deposit made when placing the order.
- Proportionally to the next payment of Goods to be taken.
- RENT - Equipment Rental minimum period of 2 (two) months in advance and paid for 2
(two) months and a deposit made at the time of placement.
- The next payment is received 15 days after the bill.
- Extension of Rental Equipment shall be made new PO / new contractand follow
the terms & Conditions -conditions apply.

- Payments made by bank transfer to BPW.

- Payment by check or Giro Bilyet be paid off after disbursed by the Bank.
Supervision of service:
- BPW provides supervision services where accommodation and transport costs will be charged to the
customer.
- Supervision service fees will be charged to customers for services that exceed the provisions of supervision.
Other requirements are not listed in this offer, contained in the document Terms and Conditions of Sale and
Purchase / Lease BPW (attached).
