Magical Mishaps
The price of playing with eldritch power
Everyone makes mistakes, and when you’re playing with the very fabric of the universe,
sometimes those mistakes have surprising, harmful, or spectacular consequences. Use
these tables for wild magic surges, effects of casting in an area that crackles with
magical energy, or any other situation you see ퟷ�t. Two tables of 100 effects each are
provided, and cheat sheets for relevant monsters and spells are also included. Note that
the spells are abbreviated for their inclusion here as chaotic side effects, and do not
necessarily re៹�ect how they would behave when cast intentionally and with care.
Magical Mishaps I
1. Roll on this table every round for 1 minute 51. Spectral shield, +2 AC, no magic missile for 1 minute
2. Caster and target switch positions after spell 52. Caster gains 3’ prehensile tail for 1 hour
3. Can see invisible creatures 53. Cannot get drunk for 5d6 days
4. A ៹�aming horse appears (Not Nightmare, real horse) 54. All unlocked doors/windows in 60’ ៹�y open
5. A modron appears within 5 feet for one minute 55. Hair falls out, grows back within 24 hours

8. Weapons in 60’ radius turn to food for 1 minute 58. Geyser lifts caster 50’ in air until start of next turn
9. Cast magic missile as 5th-level spell 59. Regain lowest-level expended spell slot
10. Caster breathes 30’ ퟷ�re cone next time they speak 60. A confused bear appears within 60’ for 1 minute
11. Change height by 1d10: Even: grow, Odd: shrink 61. For 1 minute, can only shout when speaking
12. Target is cocooned in crystal until someone breaks it 62. Grass instantly sprouts to 3’ tall within 20’, 1 minute
13. Cast confusion centered on self 63. Cast fog cloud centered on self
14. Cast thunder wave centered on self 64. Caster can only breathe underwater for next minute
15. Regain 5 HP every round for 1 minute 65. Up to 3 creatures of choice in 30’:4d10 lightning dmg
16. Target is transported to Ethereal Plan for 1 minute 66. Caster falls Unconscious until start of next turn
17. Grow long beard made of feathers until sneeze 67. Frightened by nearest creature until end of next turn
18. Swarm of rats carry caster 30’ in random direction 68. If target dies in next minute, its ghost haunts caster
19. Cast grease centered on self 69. Everyone in 30’: invisible for 1 minute or till atk/cast
20. Creatures within 30’ are stuck in place vs Str DC 15 70. All within 60’ saves vs Wis 15 or drops w/e they hold
21. For 1 minute, spell targets have disadv. on saves 71. Resistance to all damage for next minute
22. A mature oak sprouts in an unoccupied space in 60’ 72. Everything within 20’ pulled 10’ toward caster
23. Caster’s skin turns vibrant blue till remove curse 73. Random creature within 60’ is poisoned for 24 hours
24. Caster can only speak to animals for 24 hours 74. Caster smells like lavender for 1d6 days
25. Third eye grows; adv. on Perception for 1 minute 75. Glow brightly for 1 minute. Blind others within 5’.
26. Caster gains 100 lbs, move speed halved, 1 minute 76. Casters clothes become uncomfortably tight
27. Spells cost additional bonus round cast time 77. Cast polymorph on self. Fail save: Sheep form.
28. Next turn caster takes no action, vomits 1d100 SP 78. If spell would kill target, target’s extremities ៹�y apart

34. Caster can mimic target’s voice perfectly for 24 hrs 84. Loud voice ridicules caster on init count 20 for 1 min
35. Age changes 1d10 years. Odd: Younger, Even: Older 85. Cast mirror image on self
36. Caster grows antlers, sheds them in 24 hours 86. Caster’s arms become tentacles for 1 minute
37. 1d6 ៹�umphs appear for 1 minute in 60’; scared 87. Cast ៹�y on random creature within 60’
38. Caster and target ៹�ung 10’ in opposite directions 88. Large ៹�oating eye follows caster for 1 hour
39. Regain 2d10 HP 89. Become invisible/silent for 1 minute or till atk/cast
40. Gravity reverses in 30’ radius till start of next turn 90. Caster’s INT and STR swap for 1 hour
41. Become potted plant until start of next turn 91. If you die in next minute, come back per reincarnate
42. Caster distracted by cloud of gnats for next minute 92. Caster sees everyone as a decaying corpse for 24h
43. For next minute, teleport up to 20’ as bonus action 93. Size increases by 1 size category for 1 minute
44. Spell ricochets off target to random creature in 30’ 94. All ퟷ�res within 60’ radius extinguished
45. Cast levitate on self 95. You and all in 30’: vulnerable to piercing dmg, 1 min.
46. Caster’s money cycles: CP -> SP -> GP -> CP 96. Caster becomes frightened of a color for 1 hour
47. Unicorn appears within 5’ for next minute 97. Surrounded by faint ethereal music for 1 minute
48. One of target’s eyes replaced by 500 GP sapphire 98. Caster suffers a head cold for 24 hours
49. Cannot speak, emit pink bubbles instead 99. Regain all expended Sorcery Points
50. Smoke fumes from caster’s ears for 1 minute 100. Next phrase spoken by caster becomes true
Magical Mishaps II
1. Roll two more times. All effects happen at once. 51. A terrible stench suffuses the area in 50’ radius
2. Wall of force appears in front of caster 52. Sustained harmonious chord plays, audible 500’ rad
3. Caster smells like a skunk for duration of spell 53. Caster gains 8 temporary hit points
4. Caster’s hair grows one foot in length 54. Spell deals maximum possible damage, if applicable
5. Caster drops one size category for 1 hour 55. Ground shoots up in spikes to surround the caster
6. Wall of Fire encircles caster 56. Spell’s duration is doubled, if applicable
7. Caster suffers same spell effect as target 57. Spell ퟷ�res in random direction if range is not Touch
8. Caster blinded and invisible for one minute 58. Caster grappled by spectral tentacle for 1d4 rounds
9. Caster’s eyes glow like lanterns for 24 hours 59. Target’s teeth fall out
10. Whirlwinds whip around caster until next turn 60. Target is petriퟷ�ed for 1d4 rounds
11. Spell functions, any applicable saving throw auto-fail 61. Any ferrous material on target immediately corrodes
12. Spell fails but it and spent slot are retained 62. 5’ radius of caster air turns to water, splashes down
13. Spell appears to fail but occurs 1d4 rounds later 63. Caster’s arms fade into the Ethereal for 1d4 rounds
14. Caster and all within 10’ heal 1d8 HP 64. Caster a៛�icted with pica for 1d6 days
15. Target is stunned for one round 65. 5 gallons of sand appear between caster’s hands
16. Caster suffers hiccups for 1 hour (as poisoned, loud) 66. Spell ퟷ�zzles, nothing happens
17. Caster and target vanish to Ethereal Plane, 1 minute 67. Caster’s hands encased in bricks of hard clay
18. Caster’s movement leaves color trails for 1 minute 68. Caster’s tongue triples in length for 1d12 hours
19. Caster’s skin transparent for 1 round 69. Anyone in 30’ radius checks Concentration, if needed
20. Every step causes tremors, shaking nearby 70. Caster’s carnium transparent till remove curse
21. Swarm of centipedes burst forth from caster’s skin 71. Caster casts color spray simultaneous with spell
22. Caster’s move speed 2x for 1 hr, then 1 lvl exhaustion 72. Exposed fragile objects in 30’ radius shatter
23. Imbued with extra conퟷ�dence, Adv. on Charisma 24h 73. Caster becomes voraciously hungry
24. Cloud of smoke explodes from hands, lasts 3 rounds 74. Caster’s bones are gold. Spd 5’ till remove curse
25. Spectral audience of 1,000s appears to observe spell 75. Caster suffers acute amnesia (stunned) 1d4 rounds

45. Casters hair bursts into ៹�ame, 1d6 ퟷ�re dmg 95. Ferrous items < 500 lbs in 30’ pulled toward caster
46. Caster suffers extremely violent thoughts very brie៹�y 96. Target driven mad, stunned, Wis save every hour
47. Caster’s face breaks out in acne for 1d6 days 97. Caster doubts reality 1d6 hours, stunned 1d6 rounds
48. Caster’s hair turns bone white (future hair has color) 98. Target ages 1d100 years, possibly dying of old age
49. Caster gains Truesight for 1d6 days 99. Target takes an additional 6d6 force damage
50. Caster’s hands burned. 1d6 dmg, painful for 1 day 100. Spell reversed, if reverse is possible

Telepathic Shroud: Immune to mind Legendary Actions: 3 actions, take 1 at
reading and divination spells end of others’ turns. Regained on own.
Hooves: Makes one attack w/ hooves.
Tendrils: MWA, +4, 1d4+2 p dmg + 1d4 Shimmering Shield: Costs 2 actions. On
acid dmg. DC 10 Con save else take self or other w/in 60’. +2 AC till end of
1d4 acid dmg per turn till success. unicorn’s next turn.
Lesser restoration also ends effect. Heal Self: Costs 3 actions, 2d8+2 HP.

less than 16. heavily obscured for duration or till wind disperses.
Calm Emotions: 60’ range. Humanoids in 20’ rad of Grease: 60’ range, 10’ square, 1m Di៛�cult terrain. Enter,
point in range make Cha save, failure causes one of exit, end, or exist where cast: Dex save else fall prone.

1 minute, OR, make a hostile target indifferent for up to Levitate: 60’ range, 10m. Rise up to 20’. Can control
1 minute. altitude but push/pull for lateral motion. Landing=safe.
Color Spray: Blinds 6d10 HP of creatures in ascending Magic Missile: 120’ range. 7 darts (3 base + 4 more for
order in 15’ cone. 5th level), 1d4+1 force dmg each. Can strike 1-3 targets.
Automatic hit.
Confusion: 90’ range, 10’ rad sphere. Wis save else roll
d10 every turn for 1 minute: 1: Use full move in random Mirror Image: 1 min. 3 dupes appear. Shatter when hit.

CR <= level. Stats/HP replaced by new form’s stats, but
Detect Evil/Good: Self, 30’ range. 10 min. Know if retains personality/align. Drop to 0 HP breaks spell.
aberration, celestial, elemental, fey, ퟷ�end, undead, Excess dmg carries over. Can’t cast spells, speak, or use
consecration or desecration w/in range. equipment.
Dispel Evil/Good: Self, 1 min. Undead and non-Prime Remove Curse: Won’t actually remove curse from

Charm, Frighten, or Possessed by one of those

Druidcraft: Choose one: Predict 1 day’s weather with
harmless effect; Make a ៹�ower blossom or similar; 5’ Wall of Force: 10 min. 10’ x 10’. When appears,
cube of natural sensory effect (like skunk odor); light or creatures pushed to either side. Nothing may pass thru,
snuff out a candle, torch, or campퟷ�re. immune to all dmg, can’t be dispelled by dispel magic.
Also blocks ethereal travel. Disintegrate destroys it.
Entangle: 90’ range, 20’ square: Plants cause di៛�cult
terrain. Creature in area when cast makes Str save else
Restrained. Str check vs. spell save DC frees the
creature.
Fireball: 150’ range, 20’ rad sphere. 8d6 ퟷ�re dmg, Dex
save for half damage.
Fly: Touch range, 10 min. Target gains 60’ ៹�y speed.
