ABSTRACT (Sample)

Background: Pakistan like all developing countries, has trained health care providers to meet

their health care needs and to achieve the Millennium Development Goals (MDGs).

Unfortunately, these trained health care providers are leaving the country for better

opportunities. The more developed countries, such as United Kingdom, USA, Canada, and

Australia are facing a shortage of trained health care providers; therefore, these countries

attract them from developing countries, causing depletion in the human resources for health

care in the source countries.

Objective: The research project aims to identify the factors responsible for Pakistani trained

health care providers moving abroad and to draw together recommendations for retaining

them, from the respondents, through a questionnaire - based study, in selected government

and private tertiary care hospitals in Karachi, Pakistan.

Method: A cross sectional descriptive survey was conducted in two teaching hospitals of

Karachi, Pakistan. A sample of 150 trained health care providers was taken randomly, using

the lottery technique, in teaching two selected government and private hospitals, which

were demographically situated in different parts of Karachi and serving a diversity of

population. To gather data on demographic variables and reasons for moving abroad, a

questionnaire which was pilot tested and validated, was administered to the selected

participants. Data were analyzed using frequencies, percentages and ranking, and were

interpreted using Maslow’s Theory of Hierarchy of Needs as a framework.

Results: The results showed that low salary package, unsatisfactory working conditions were

the major reasons why trained health care providers wanted to leave the country and work

or migrate abroad. The United Kingdom, Canada, and the United States of America are the

most popular countries where Pakistani trained health care providers would like to work and

migrate. Recommendations made by the respondents for staying back and working in

Pakistan, relate to the reasons why they want to move abroad. If something can be done to

the handsome packages of trained health care providers, the safety and security of the

professional workers, and the working conditions, then they would stay and work in

Pakistan. The results have deep implications for health leaders and the government to work

for the fulfillment of the health profession in the country.

Conclusion: The reasons why Pakistani trained health care providers are moving abroad are

linked to the fulfillment of basic human needs, self esteem, safety, and security needs. These

issues are similar to the reasons why those in other source countries also want to migrate

and working in the developed countries. Policy makers of developed and developing

countries should address the impact of migration on the humane source countries as well as

on their health service delivery systems.
