    TO : SPO1 Brian Ruffin Cortez
    SPO2 Bernard Balonzo Palanca

    FROM : Officer-In-Charge

    SUBJECT : Explanation

    DATE : August 20, 2018


    1. This has reference to the inspection conducted by the undersigned in your
    respective designated Fixed Visibility Point in Pasong Buaya cor Daang Hari Rd., Brgy
    Pasong Buaya II and Aguinaldo Highway cor Daang Hari Rd., Brgy Anadu II-D at about
    1:30 PM of August 20, 2018, wherein it has noted that there was no Patrol Personnel
    present during the inspection in the aforesaid areas considered as Fixed Visibility
    Points.

    2. In this regard, you are directed to explain within 24 hours upon receipt
    hereof why no disciplinary action should be taken against you of your failure to
    supervise your patrol personnel.
