The Risk Assessment Project is broken down into a number of stages, which should flow from start to finish, telling a complete story. Begin with describing the process, activity or task to be risk assessed. Then identify three hazards, assessing the risks associated with those hazards and recommending appropriate risk controls. We've provided you with: = a two-page Risk Assessment form on which you should record your project. = aguidance page, including a Risk Rating Calculator, Action Level Table and an example Risk Assessment (for you to refer to when completing your project). Please complete each section of the Risk Assessment form as detailed below: 4.
