Dear Fellow Medial Staff,

I am the Service Line Director for Total Knee and Hip Arthroplasty.
There has been a significant advancement of both technology and
medical care for this field. From Robotic (Makoplasty) surgical
navigation to peri-operative anesthetic pain management and
medical clearance, the field is progressing rapidly.
My principle goal is to advance patient care centered around total
hip and knee arthroplasty for the William W. Backus Hospital.
Currently, we are focusing on the Joint Commission Center
of Excellence Certification (maintenance), and the Medicare
Comprehensive Care for Joint Replacement (aka “The Medicare
Bundle”).
William W. Backus hospital was certified as a Center of Excellence
by The Joint Commission for Total Hip and Knee Arthroplasty in
August 2017. The purpose of this certification is to validate the work
being performed day-in and day-out by the entirety of the hospital
and surgical staff. We monitor the arthroplasty service line and
patients to improve outcomes and reduce complications. Our current
four monitored metrics are:
[1]1 Allogenic blood transfusion rate

2 Discharge to home rate

3 Out of bed POD #0 rate
[4]
4 Pre-operative educational class attendance rate.
We are currently making progress on these metrics, as depicted in
the following slides.
Comprehensive Care for Joint Replacement was initially a
mandatory program started by Center of Medicare and Medicaid
Services. The main goals were to improve patient outcomes and
increase efficacy of patient care. This is to be accomplished by
decreasing non-valued added expenditures in the global period.
Bundle payment operates as follows. The hospital is the bundle
initiator. All Medicare total hip and knee arthroplasties are
performed at Backus Hospital, and all medical expenditures within
90-day period are included. Medical expenditures include in-patient
hospital expenses, skilled nursing care, at-home care, in-office
visits, DME, physical therapy, imaging studies, laboratory tests, etc.
Backus Hospital could gain or lose money depending on overall
expenditures. This is modified for complication rates and whole
hospital HCAPS scores.
We are making progress on the above program goals. Please
