Fatality Summary

Based on the information obtained during the investigation, the Department of Children and
Families, Child Protective Investigations closed the abuse investigation on August 6, 2018, with
no indicators of death due to inadequate supervision. While the father did make a poor parenting
decision to sleep with his child noting he had been told by medical professionals about the risk of
co-sleeping, this decision does not rise to the level of neglect due to inadequate supervision.

2. Other Children in the Family

had three older siblings at the time of her death. These children were all determined to be
safe in the care of their parents. The family was referred for grief counseling but did not engage
with this service.

3. Summary of Prior Agency Involvement with Family

The family does not have any prior abuse report history with the department.
