AFFIDAVIT OF LOSS
I, RODOLFO FUENTEBLANCA AGUILAR, JR., of legal age,
Filipino, married and with address at Summerville, Pinagmisahan, Antipolo,
Rizal, after having duly sworn to in accordance with law, hereby depose and
say that:

1. I was in possession of a passport issued by the Bureau of

Immigration and Deportation (BID);

2. Said passport was kept inside a brown envelope and I carried it

with me last September 10, 2018 when I went to Manila;

3. Unfortunately, said passport was discovered missing from my

envelope and it probably slipped off said envelope unnoticeably
while I was at our above-given location;

4. I have exerted all efforts to locate find said receipt but to no avail
and it is now considered lost and/or gone for all intents and
purposes;

5. I have executed this affidavit to attest to the foregoing facts and for
whatever lawful purposes it may serve.

I, CATHERINE G DELA CRUZ, of legal age, Filipino, married and
with address at 1008 Baltimore Street, Brookside Hills Subdivision, Cainta,
Rizal, after having duly sworn to in accordance with law, hereby depose and
say that:

6. Last May 2, 2018, I purchased some items from Mandaue

Furnitures, Cainta, Rizal for which I was issued a receipt;

7. Said purchase receipt was kept inside my wallet for safekeeping;

8. However on July 13, 2018, said receipt was discovered missing

from my wallet and it probably slipped off said wallet unnoticeably
while I was at our above-given address;

9. I have exerted all efforts to locate find said receipt but to no avail
and it is now considered lost and/or gone for all intents and
purposes;

10. I have executed this affidavit to attest to the foregoing facts

and for whatever lawful purposes it may serve.
