Faktor penyebab ketidaktepatan kode:

Pengkodean yang tidak tepat disebabkan oleh pengkodean tidak dilakukan oleh perekam
medis yang memiliki kompetensi kodefikasi dan klasifikasi penyakit. Penulisan kode
penyakit di berkas rekam medis dilakukan oleh dokter, sedangkan penulisan kode diagnosis
di SIMPUS dilakukan oleh perawat. Daftar kode diagnosis di dalam SIMPUS belum lengkap
seperti ICD offline sehingga diagnosis yang tidak ada kodenya akan dikode dengan kode
yang mendekati sehingga terjadi ketidaktepatan pengkodean. Untuk kode diagnosis yang
tidak terisi disebabkan tidak ada kewajiban/aturan penulisan kode di dalam berkas rekam
medis dan tidak ada kolom khusus untuk mengisi kode tersebut. Pengkodean langsung
dilakukan ketika mengentri data ke SIMPUS.

Oleh karena itu sudah seharusnya kode penyakit di SIMPUS harus segera dilengkapi,
mengingat pengkodean diagnosis dilakukan langsung di dalam SIMPUS dan bukan perekam
medis yang melakukan pengkodean tersebut sehingga riskan terjadi kesalahan pengkodean.
