6. That his name and the date of his birth reflected in the Commission on Elections
(COMELEC) ID and Senior Citizen ID issued by OSCA-Rizal, Nueva Ecija, as MARIO A.
MARAVILLA and DECEMBER 9, 1951, respectively, as per copy of his Voter’s ID and
Senior Citizen ID card as Annexes “B” and “C”, respectively;

7. That we were surprised to find out when he showed to us his NSO Certification where
his name and date of birth reflected as MARIO A. MARAVILLA and DECEMBER 8, 1952,
respectively as per copy of NSO Certification attached and marked herein Annex “D”;

8. That the person named in his Certificate of Birth, Voter’s ID, and Senior Citizen ID as
MARIO A. MARAVILLA born on December 9, 1951 and in the NSO Certification as MARIO
A. MARAVILLA born on December 8, 1952 is one and the same person;

9. That we are executing this Affidavit to attest to the truthfulness of this fact and to
support his request for the Benefit Claim for the GSIS and for whatever legal intents and
purposes.

family friend and town mate;

26. That we know for a fact that PURIFICACION L. CRUZ died on April 09, 2007 in Sta.
Rosa, Nueva Ecija;

27. That as per certificate from the National Statistics Office, they found out that
PURIFICACION L. CRUZ has no record of death as per attached certificate
marked herein as Annex “A”;

28. That this Affidavit is to attest to the truthfulness of this fact and to record with the
Local Civil Registrar of Sta. Rosa, Nueva Ecija and with the National Statistics
Office the fact of death of the late PURIFICACION L. CRUZ and for other legal
intents and purposes.

2012 at Dr. PJGMRMC, Cabanatuan City, Nueva Ecija as per attached his Death

2. That my father was a member of the Social Security System (SSS) when he was still
living;

3. That as his son, I am the designated beneficiary on his Social Security System (SSS);

4. That I have already claimed the burial benefit from the Social Security System (SSS) for
my father’s death;
