2. Personal Wealth Audit (Form 4)
3. $200 Cash ($50 note x 3, $10 note x 5) (Note: Cash is used as a prize money in Millionaire Game and is strictly non-
refundable)

Payment
 All Payment is non-refundable and non-transferable across programs.
 Full payment must be made before the program otherwise there is no guarantee of seat.
 Full payment is inclusive $500 deposit
 Crossed cheques should be made payable to ‘ Adam Khoo Learning Technologies Group Pte Ltd ‘
 An administration fee of S$200 will be charged for cancellation of 0% interest-free credit card instalment plan.
 Deposit is not refundable

Attendance
 No-notice/No-show will be deemed as withdrawn and refunds will not be allowed.
 Postponement notice must be given via fax (65- 62742105) or email (wealthacademy@akltg.com) 14 days prior to commencement date, otherwise,
administrative fee of $200/- will be imposed.
 Each applicant is only allowed to defer his/her attendance twice after which the deposit will be forfeited.
 No cancellation is allowed 14 days prior to commencement date, but a replacement is welcomed with written notice.

General
 Incomplete forms will not be processed.

Entitlements
 Each applicant is entitled to 2 complimentary books. (Applicable for ‘Secrets of Self-Made Millionaires’ only)
 Entitlements are not exchangeable for cash.
 Participant who paid spouse price/student price will not entitle for 12 SSMM Audio CD set
