Resumen
Objetivo: analizar el uso de la Acupuntura en los pacientes con cefalea cluster.
Contexto: la Acupuntura se ha vuelto muy popular en USA y Europa, y está entrando con fuerza en
Ecuador. Esta terapia ha demostrado excelentes resultados ante diversas enfermedades, requiere
instrumentos sencillos, es segura y económica y de fácil aprendizaje y no ofrece resultados negativos.
Su difusión es amplia en el pueblo chino, y también se ha ido ganando progresivamente la confianza
y consideración de los diversos países del mundo. Aunque la cefalea en racimos es la forma más
incapacitante de cefalea primaria, en cuanto a las terapias alternativas y complementarias existen
evidencia disponible sobre la efectividad de la Acupuntura como tratamiento. Los puntos más
utilizados son ExHN-5 Taiyang, GB-14 Yangbai (solamente en el lado afectado), GB-20 Fengchi (en
ambos lados), LI-4 Hegu, LR-2 Xingjiang, SP-6 Sanyinjiao, ST-36 Zusanli (todo en ambos lados). En
cada punto, después de la inserción de la aguja, la sensación de «De Qi» fue evocado; después de
obtener esta sensación los puntos de acupuntura no fueron estimulados además por un período de
20 minutos, hasta su extracción.
Discusión: el mecanismo de acción de la terapia de acupuntura es complejo. Algunos estudios han
demostrado que la electro-acupuntura puede aumentar los péptidos opioides endógenos como
encefalinas y betaendorfina, en las regiones supraespinales del SNC y en la médula espinal, mientras
que la acupuntura manual puede conducir a la activación de los controles inhibitorios nocivos difusos,
con una supresión inmediata de la transmisión del dolor en las neuronas del trigémino caudalis y/o
la médula del asta dorsal. Algunos estudios bioquímicos evidenciaron niveles significativamente
menores de meta-encefalina en el LCR, y beta-endorfinas en sangre periférica. Los autores especulan
que estos hallazgos reflejan reducidos niveles del SNC de beta-endorfina debido a hipofunción del
sistema opioide. Por otro lado, los niveles de opioides pueden aumentar después de la acupuntura
manual o electro-acupuntura. Conclusión: se recomienda la Acupuntura como terapia alternativa
en los pacientes con cefalea en racimos.
Abstract
Objective: To analyze the use of acupuncture in patients with cluster headache. Background:
Acupuncture has become very popular in USA and Europe and entering with force in Ecuador. This
therapy has shown excellent results against various diseases, requires simple instruments, it is safe
and inexpensive and easy to learn and offers no negative results. Its spread is wide in the Chinese
people, and has gradually been gaining the trust and consideration of various countries of the
world. Although cluster headache is the most crippling form of primary headache, as to alternative
and complementary therapies are available evidence on the effectiveness of acupuncture as a
treatment. The most frequently used points are ExHN-5 Taiyang, GB-14 Yangbai (only on the affected
side), GB-20 Fengchi (on both sides), LI-4 Hegu, LR-2 Xingjiang, Sanyinjiao SP-6, ST-36 Zusanli (all sides).
At each point, after insertion of the needle, the sense of “De Qi” was evoked; after obtaining this
feeling acupuncture points they were not further stimulated for a period of 20 minutes, until extraction.
Discussion: The action mechanism of acupuncture therapy is complex. Studies demonstrated that
electrical acupunture can increase endogenous opioid peptides (enkephalin, betaendorphin) in
supraspinal CNS regions and in the spinal cord, while manual acupuncture can lead to the activations
of the diffuse noxious inhibitory controls (DNIC) with an immediate suppression of pain transmission in
neurons of the trigeminal caudalis and/or the spinal dorsal horn. Some biochemical studies evidenced
significantly lower met-enkephalin levels in CSF and lower peripheral blood beta-endorphin. Some
authors speculated that these findings reflect reduced CNS levels of beta-endorphin due to an opioid
system hypofunction. On the other hand, CSF opioid levels may rise following manual acupuncture
or electroacupuncture.
Conclusion: Acupuncture is recommended as an alternative therapy in patients with cluster
headache.
