JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, SUSANA HERNANDEZ and LOWEL NAVARRO, both of legal ages,

Filipino and presently are residing at Sta. Cruz, Sto. Tomas, Batangas respectively,
after having been duly subscribed to in accordance with law, hereby depose and state
THAT;

1. We personally know EDWIN A. MORCILLA since we were

neighbors/acquiantance for a long time;

2. We likewise know that EDWIN A. MORCILLA is a member of the Social

Security System with number 33-2537395-6;

3. We likewise know that there exists a discrepancy in the records of EDWIN A.

MORCILLA before the Office of the Social Security System; that said discrepancy is
in his year of birth which was erroneously entered as “1974” instead of the true and
correct year of birth as “1972” as evidenced by herein attached copy of Certificate of
Live Birth issued by the Philippine Statistics Office, marked as Annex “A”;

4. THAT the name EDWIN A. MORCILLA appearing in the records of the Social
Security System and the EDWIN A. MORCILLA appearing in the above birth
certificate refers to one and the same person;

5. That we are not related to said couple or their child either by blood or affinity;

6. We are executing the foregoing affidavit to attest to the truthfulness of the

foregoing facts and for all other legal intents and purposes.
