1 PURPOSE
The purpose of the present Quality Plan is to describe the measures put in place for managing
Quality, by N. Loukatos & Associates SA to carry out the project “Geotechnical Investigations and
Design for structures for the part Patras – Lechaina of the new highway Elefsina – Korinthos –
Patras – Pyrgos - Tsakona Motorway (EKPPT)” within the framework of execution of the EKPPT
Project, for “VINCI Construction - Grands Projets”.
Quality is one of N. Loukatos & Associates core values – we believe it is the responsibility of
everyone in our team. As such, our Quality Assurance Plan / Quality Control Programme (QA/QC)
is proactive and designed to ensure that all project activities – from preliminary investigations to
projects hand over – comply with contract, design, workmanship, and performance requirements.
N. Loukatos & Associates group of companies (N. Loukatos & Associates S.A, GeoSTAND Co
and STAND Engineering Consultants S.A.) has established a Quality System conforming to the
requirements of BS EN ISO 9001:2000 and is certified to its application. Moreover N. Loukatos &
Associates Soil and Rock Mechanics and Quality Control Laboratory is operating and is certified
to operate according to the EN ISO 17025 standard, and its valid Quality Management Manual
has been compiled accordingly and approved by the Greek Ministry for Environment - Physical
Planning and Public Works (KEDE) and the Hellenic Accreditation System S.A. (ESYD).
QA/QC begins at project start-up with development of the design; because we view quality not as
the result of tests and inspections but as a complete process. QA/QC means ensuring that all
equipment, materials and workmanship meet or exceed expected levels of quality and
performance.
The QA/QC responsibles will prevent deficiencies and non-conformance by monitoring the
activities that affect quality and verifying and documenting the achievement of quality in technical
work through regular meetings and inspections. The team will record objective evidence to
confirm that contract requirements have been and continue to be met.
2 PROJECT OVERVIEW
N. Loukatos & Associates has been awarded from “VINCI Construction - Grands Projets” the
execution of geotechnical investigation and the geotechnical design of the structures of the part
Patras to Lechaina part of the “Elefsina – Korinthos – Patras – Pyrgos – Tsakona Motorway”
(“EKPPT”).
Patras – Lechaina section is located in the northwestern Peloponnese. The Patras - Lechaina
section is a widening of the current road on the first 15kilometres and then a new alignment.
The main issues of this section are overpasses, underpasses, bridges over rivers and culverts.
The project is constituted by:
A. In situ works and test:
Destructive drillings with parameters recording, in order to carry out Menard
pressuremeter tests.
Core boreholes, coupled with standard penetration test.
Piezometers setting and monitoring.
B. Laboratory tests:
Moisture content tests, bulk and dry density determination.
Soil classification tests including particle size distribution (sieve and hydrometer analysis),
Atterberg limits, methylene blue value determination and definition of organics content.
Rock classification tests including Petrographic and Mineralogical analysis, Rock density
(γd) and CaCO3 % definition tests.
Soils mechanics tests including oedemeter tests, swelling tests, drained shear strength
tests (Casagrande box), consolidated and undrained, shear strength tests in the ring
shear machine, unconsolidated undrained triaxial tests (UU), consolidated undrained
triaxial tests with pore pressure measurement (CUPP) and consolidated drained triaxial
tests (CD).
Rock mechanic tests including point load tests, Brazilian tests, uniaxial compression
strength (UCS) with and without determination of modulus of elasticity and Poisson ratio,
Micro Deval tests (MDE), Los Angeles tests (LA), Fragmentability tests (FR), Degradability
tests (DG), Cherchar abrasiveness tests, discontinuities shear strength tests.
Material reuse tests including normal Proctor test, immediate bearing index (IPI), soaked
California Bearing Ratio (soaked CBR) and soils treatmentability tests.
C. The carry out and supply of different type of reports:
On site factual reports,
Geological description of boreholes logs,
Laboratory tests reports,
Materials reuse studies.
D. The elaboration of the geotechnical design for the structures i.e.:
The determination of the ultimate bearing capacity of the soil,
The determination of the allowable bearing pressure,
The proposal of an adequate modulus of subgrade reaction (lateral and horizontal),
The calculation of arising settlements,
The calculation of bearing capacity of bored or driven piles of various diameters,
The proposal of soil amelioration measures and the calculation of resulting bearing
capacity and settlements.
3 ORGANISATION
The project “Geotechnical Investigations and Design for structures for the part Patras to
Lechaina” of the “EKPPT” motorway is regarded by our group of companies as a major project
and will therefore be managed by the higher management of the company, i.e. Messrs Nestor
Loukatos, General Director of N. Loukatos & Associates and Gerasimos Loukatos, General
Director of geoSTAND.
Specialists will lead the development, implementation, and regular review of our internal QA/QC
systems and formal testing programme, with the QA/QC team being actively involved in the
monitoring of the testing programmes.
Mr. Georges Vatsellas, Civil Engineer, will be acting as Project Manager for the Project
Mr. Christos Bourounis, Geologist, will be acting as Head of Site Operations for the Project
Mrs. Eirini Boutsikou, Mining Engineer, will be acting as Quality Representative
Mr. Peter Arber, Civil Engineer and Geologist, will be acting as Safety Manager
Mrs. Anastasia Sotiropoulou, Geologist and Geotechnical Engineer, will be responsible for reports
and design submission
Mrs. Athina Pliaka, Geologist, is the Head of geoSTAND’s Soil and Rock Mechanics Laboratory
Staffing structures have been devised to meet the needs of the project. Employee skills and
competency levels have been matched to carefully formulated job profiles. Competency
assessment, which is an ongoing process, will allow the identification of further training and
development needs.

4 CONTRACT REVIEW
We will utilize a project quality team approach for the investigation, laboratory testing and design
quality control. This includes a management and quality plan, which is presented herein.
The first point of our plan, the preparation of the project, includes means to ensure client’s
requirements understanding compliance and client’s satisfaction.
The Project Manager assigns the head of sections (Quality Representative, Safety Manager,
Head of Site Operations, Head of Reports and Design Section) based upon the personnel’s
knowledge and experience to assure every individual is capable of performing the assigned
function.
In a common preparatory meeting the PM and the heads of sections will:
g. Assure that all personnel, equipment and materials to fulfil the project requirements are
available.
h. Identify personnel’s training needs and plan QC, Environmental protection and H&S
audits.
i. Assure that required Quality Control testing will be provided and establish an Inspection
and Testing schedule.
l. Identify any other preparatory steps dependent upon the particular operation.
5 COMMUNICATION
N. Loukatos & Associates is accommodated in one, purpose made, office and laboratory building,
the internal communication between the sections (design and laboratory) is therefore facilitated.
The internal communication is performed verbal, or, on important items, by internal notes in
writing.
Whenever necessary meetings of the management and the section heads are called the minutes
of the meetings and decisions are recorded in the forms foreseen by the company’s Quality
Control Plan, and distributed to the participants or to whomever else is thought necessary in
writing. Decisions concerning the staff are also posted on special announcement boards, visible
and accessible by all staff.
The communication with the workshop and the depot is ensured by phone, if needed written
reports are provided, upon management’s decision.
The communication with the various, multiple and, usually of short duration, working sites is
ensured through the construction supervisors.
N. Loukatos & Associates always requires a qualified field geologist or geotechnical engineer to
supervise operations, collect samples, perform in situ tests, record progress and results and
provide a detailed geologic description of the formations encountered in each borehole drilled or
pit opened. The construction supervisors are all equipped with company’s mobile phones to
enable them to communicate immediately with the project management and also with laptop
computers with mobile internet access to be enabled to transfer reports, records and
photographs. On major sites fax machines are also installed to facilitate communications
furthermore.
The communication with the on site supervisors, passes through the Head of Site Operations and
is communicated to the PM and the management.
In addition to the above, inspections and meetings on site and in the office will be held, whenever
deemed necessary, to ensure the fulfilments of the projects time schedule and QC, Environmental
protection and H&S requirements. The minutes of the meetings and decisions will be recorded in
the forms foreseen by the company’s Quality Control Plan, and distributed to the participants or to
whoever else is thought necessary in writing.
The communication with the client’s representatives will be maintained by the following ways:
1. The communication between the client and N. Loukatos & Associates will be maintained
by written reports, submittals, e-mails and phone calls.
2. Meetings and joint site visits will ensure the fulfilments of the projects time schedule and
QC, Environmental protection and H&S requirements. The minutes of the meetings and
any decisions will be recorded and distributed to the participants in writing.
These follow-up meetings and joint site visits will be carried out at least every three
months.
The client and / or N. Loukatos & Associates will call additional meetings whenever
deemed necessary. Unannounced site visits can be performed either by the client’s,
Independent Engineer’s, governmental agencies or N. Loukatos & Associates
representatives whenever thought necessary, without giving prior notice to N. Loukatos &
Associates management. Any non compliance reports, remarks etc will have to be
communicated to N. Loukatos & Associates in writing.
3. The site supervisors of N. Loukatos & Associates are instructed to obey to verbal
instructions or remarks from the client’s site supervisors or governmental agencies. They
are instructed to communicate all instructions and remarks received immediately by
phone, and if asked for in writing, to the Head of Site Operations.
N. Loukatos & Associates site personnel; borehole crews consisting of a supervisor (geologist,
geotechnical engineer or drilling foreman, driller, assistant and occasionally unskilled assistants,
drivers and mechanics) will be working over every second weekend (Saturday and Sunday) and
will not be working the following one.
Due to the fact that more than one crew will be working on the project at the same time it is not
feasible to state that all works will be halted over every second weekend, works may proceed
continuously while certain crews take their leave.
N. Loukatos & Associates is aware of the laws and regulations in Greece regarding weekend and
overtime work and will obtain all necessary permits from the local authorities to be enabled to
maintain the foreseen work schedule and complete the project in time.
The company is dedicated to a quality policy that will ensure that its products and services fully
meet the requirements of its customers at all times. The goal of the company is to achieve a high
level of customer satisfaction at all times. Commitment to the implementation of supporting
managerial and business operational systems is essential to realising that goal.
We will establish a Quality system for the duration of the project, which complies in general with
the requirements of EN ISO 9001:2000 Quality Management Systems.
QA/QC begins at project start-up with a review of the project because we view quality not as the
result of tests and inspections but as a complete process. QA/QC means ensuring that all
materials and workmanship meet or exceed expected levels of quality and performance.
Our QA/QC Programme remains flexible so that it can be tailored to meet the needs and local
requirements as work progresses. As required in the contract documents, details of all procedures
and compliance documents will be submitted for information before commencing each work
stage. All project phases have QA/QC as a basic core value – it will be built into everything we do.
Quality must be sought at every step in the process. To ensure that it is, each member of the
team has specific quality-related activities for which they are responsible.
The QA/QC team will prevent deficiencies and non-conformances by monitoring the activities that
affect quality during the works and by verifying and documenting the achievement of quality
through regular meetings and inspections. The team will record objective evidence to confirm that
contract requirements have been and continue to be met. QA/QC will then continue to be
undertaken as a normal part of operations.
N. Loukatos & Associates affiliated Soil and Rock Mechanics and Quality Control Laboratory is
operating and is certified to operate according to the EN ISO 17025 standard. GeoSTAND’s valid
Quality Management Manual has been compiled accordingly and has been approved by the
Greek Ministry for Environment - Physical Planning and Public Works (KEDE) and the Hellenic
Accreditation System S.A. (ESYD). To ensure quality control extends to the offsite testing and
reporting activities similar procedures have been established.
During the works our engineers will order tests, make measurements, and direct operations in
accordance with the developed specifications. The QA/QC team will verify that the tests are
complete, with results accurately reported.
8 DOCUMENTATION
8.1 Contract
The contract between “VINCI Construction - Grands Projets” and N. Loukatos & Associates
regarding the execution of the Earthworks Geotechnical Investigation and Design of the part
Patras – Lechaina of the Elefsina – Korinthos – Patras – Pyrgos Tsakona Motorway (EKPPT) has
been signed the 20th November 2008.

Wherever differences between the mentioned test methods are discovered the procedure to be
followed will be mutually agreed upon, based on engineering judgment, availability of equipment
and design needs.
The technical documents to be issued by N. Loukatos & Associates SA are the following:
9 TIME SCHEDULE
The total time foreseen for the completion of the project, with the quantities mentioned in “VINCI
Construction - Grands Projets” invitation to offer, send by email the 7th November 2008, amounts
to 168 days (24 weeks) from the day the permission to proceed with the on site works is issued.
10 HUMAN RESOURCES
The following personnel of N. Loukatos & Associates will be assigned for this project not including
project management.
The numbers given in the following table represent the maximum number of personnel of the
specific profession, which is envisaged to be engaged in the specific task.
No construction camps will be erected, the personnel on site will be housed in nearby
communities. The work areas will be supplied by N. Loukatos & Associates.
N. Loukatos & Associates will have a central maintenance and storage yard and core boxes
storage facility, rented nearby.
11 PERSONNEL TRAINING
Quality is the responsibility of everyone in our team and must be sought at every step in the
process. Our Quality Assurance Plan / Quality Control Programme (QA/QC) is designed to ensure
that all project activities comply with contract, specifications, design, and performance
requirements.
N. Loukatos & Associates engages its personnel based upon the personnel’s knowledge
and experience to assure every individual is capable of performing the assigned function.
Upon engagement every member of the design team, the laboratory team and the on site
geotechnical investigations team (geotechnical engineers, geologists, laboratory
technicians, drill rig operators) is introduced to the company’s quality policy.
Every member of N. Loukatos & Associates team is constantly evaluated according to its
commitment to perform at the highest possible quality standards and to continuously
improve its performance and deepen its knowledge. Competency assessment is an
ongoing process, which allows the identification of further training and development
needs.
N. Loukatos & Associates higher management as well as the Quality Control Representative and
the Safety Manager regularly perform site inspections and tests to assure the continuing
conformance of the work to workmanship standards. These inspections include:
The purpose of these inspections is to allow further work upon QC, Environmental protection and
H&S aspects of the work. After every inspection, the Quality Representative and the Safety
Manager prepare a list of deficient items (punch list) discovered during the inspection. The punch
list is forwarded to the management for further actions.
12.1 Identification
N. Loukatos & Associates affiliated Soil and Rock Mechanics and Quality Control Laboratory is
operating and is certified to operate according to the EN ISO 17025 standard. GeoSTAND’s valid
Quality Management Manual has been compiled accordingly and has been approved by the
Greek Ministry for Environment - Physical Planning and Public Works (KEDE) and the Hellenic
Accreditation System S.A. (ESYD).
All equipment used to perform, measure and monitor the laboratory tests is listed in the
Measuring Equipment Catalogue of geoSTAND’s laboratory, coded E2-Δ.210 as per GeoSTAND’s
valid Quality Management Manual. The catalogue is attached in the appendices.
The in situ measuring and testing equipment which will be used in the realization of the project
“Geotechnical Investigations for section 4 of the Elefsin a – Korinthos – Patra – Pyrgos
Tsakona Motorway” is listed in the table below.
Qualified personnel only will perform work during the design / report phase.
All Designs and Reports will be checked and reviewed by qualified personnel.
All Reports will be reviewed, approved, and signed by the responsible engineer or
geologist prior to submittal to the Project Manager for review and approval.
Only approved documents will be submitted to the client “VINCI Construction - Grands
Projets”.
The QA/QC activities will include comprehensive design reviews; engineering calculation
checking, drawing checking; QC audits, and other project design-related QA/QC activities at key
milestone points in the project work.
All suppliers and manufacturers of equipment used to perform, measure and monitor the
laboratory tests and the in situ investigations are evaluated according to geoSTAND’s valid
Quality Management Manual. GeoSTAND is certified to operate according to the EN ISO 17025
standard.
15 INTERNAL AUDITS
In addition to the above and to ensure compliance with QC/QA, Environmental protection and
Health and Safety the PM and the assigned Quality Representative, Safety Manager and Head
of Site Operations perform regular as well as unannounced site visits and inspections as part of
our management and quality plan.
The purpose of these inspections is to allow further work upon QC, Environmental protection and
H&S aspects of the work. After every inspection, the Quality Representative, the Safety Manager
and the Head of Site Operations will prepare a list of deficient items discovered during the
inspection and, if major deficiencies are discovered, complete a Non Conformance Report. The
list will be submitted to the PM and transmitted to the respective supervisor of the work for
estimated dates of completion for the correction of the deficient items. Upon completion of the
deficient items the Quality Representative will submit a statement to the PM stating that all work,
except as specifically noted, is complete and in accordance with the general and contract
specifications.
Follow-up inspections and tests will be carried out at least every three months, unannounced site
visits and inspections can be performed by the Quality Representative and the Safety Manager
whenever thought necessary, without giving prior notice to the PM or the Head of Site Operations.
Whenever material or equipment received for use or to be incorporated in the project are
not according to specifications
Upon receipt of an NCR the Project Manager, in cooperation with the Head of the relative Section
has to decide and implement appropriate Corrective and Preventive Actions. If deemed
necessary the higher management of the company will be informed so that the Corrective Action
is implemented immediately.
The members of the personnel connected with the NCR will be informed regarding the NCR and
the Corrective and Preventive Actions to be implemented, if deemed necessary in writing. Upon
decision regarding a Corrective and Preventive Action the Quality Representative will be informed
and approve the actions decided.
Upon completion of the actions the Quality Representative will be informed and he will review the
completeness and effectiveness of them.
The Preventive and Corrective Actions proposed and implemented will be recorded as foreseen
on the NCR sheet.
The Quality Representative is responsible to collect all NCR’s, to keep the NCR register and to
follow up any Corrective Actions needed. The Quality Representative has to present during the
regular internal audits all NCR’s and the NCR register. The Quality Representative is responsible
to follow up the implementation of any Corrective and Preventive Actions.
