MEMORANDUM

7. SUBSTANTIATING DOCUMENTS:
Affidavits of Mr. Trebuh Tnek, Mr. Remy Mendez and Ms. Bumble Bee see attached.
The O Zone Disco club fire killed 162 people. The building was totally burned and
destroyed. The origin of the fire was the spark started from DJ’s booth followed by brown-out.
According to the Remy Mendez Security of the Disco the people was already panicking when
they saw the fire. Ms. Bumble Bee also said that the fire was accompanied by heavy smoke that
caused hard breathing for them. A fast-food restaurant was affected by the fire, the damage to the
property estimated P 1.2 Million.
The required floor plan for the establishment was not followed that caused victim to be
trapped inside the disco. There is no fire hydrant nearby the establishment that caused for the fire
fighters to extinguish the fire immediately.
DISCUSSION:
Upon arriving at the Disco the fire was already consumed the most part of the
establishment. The fire responder was arduous to extinguish the fire due to limited supply
amount of water they are capable of and the lack of fire hydrant in the area. Another factor that
challenged the fire fighters was the heavy dark smoke produced by burning materials inside. The
flashover that immediately spread around the establishment gives difficult situation.
Furthermore, the cause of the fire was greatly relied upon the testimony of the witness
and victims however, it is still undetermined as the investigation is on-going.

RECOMMENDATION:
The requirements for the establishment should be in accordance always to avoid any
problems that may occur during fire incident and other unfortunate events. The critical value for
this requirement is necessary to prevent possible casualty in the future. The investigation is
profound to provide justice and punish who is accountable for the incident.
