A GUIDE TO USING THE BADAC FUNCTIONALITY SELF-ASSESSMENT AND AUDIT FORM

1. Put a check on the box that corresponds to the given questions.

e.g. Is BADAC composed of at least seven (7) members?

If your BADAC has atleast seven (7) members, put a check on the Yes Box. On the other hand, if the members consist only of six (6) and below,
put a check on No Box.

2. So as to prove the correctness of the given answers, the user/s of these forms are required to attach certain Modes of Verifications (MOVs).
Beside the Yes and No column is the list of prescribed MOVs per indicator. Put a check on every MOV attached along with the form.

5. The C/MLGOOs shall refer to the Table 1. Data Gathering and Points Distribution Matrix (page 30 of the manual) for the Pointing System.

6. The Points and Remarks box on the rightmost part of the form is intended for the use of C/MLGOO. The Points box is where the points per indicator shall be written.
Meanwhile, the C/MLGOOs may opt to put in the Remarks box any discrepancy they find in the submitted MOVs.

9. Upon determination of the total points garnered, the C/MLGOO may refer below to determine the corresponding BADAC Adjectival Rating:

85 to 100 Poin - HIGH / IDEAL FUNCTIONALITY

51 to 84 Points - MODERATE / PROGRESSIVE FUNCTIONALITY
50 Points Belo - LOW / BASIC FUNCTIONALITY
